摘 要:目的:探讨改良气管插管-肺表面活性物质-拔管(INSURE)技术治疗早产儿呼吸窘迫综合征的临床效果。方法:将2022年7月—2024年7月靖江市人民医院收治的78例呼吸窘迫综合征早产儿以随机数字表法分成两组,每组39例。对照组采用传统INSURE技术进行治疗,观察组采用改良INSURE技术进行治疗。比较两组治疗效果。结果:治疗前,两组患儿动脉血氧分压、动脉血二氧化碳分压、pH值相比,无明显差异(P>0.05)。治疗后,两组动脉血氧分压、pH值高于治疗前,动脉血二氧化碳分压低于治疗前,差异有统计学意义(P<0.05);但两组间比较,无明显差异(P>0.05)。观察组患儿肺表面活性物质反流、经皮血氧饱和度<85%发生率低于对照组(P<0.05);两组心动过缓、呼吸暂停发生率相比,无明显差异(P>0.05)。观察组气漏综合征、感染发生率较对照组低(P<0.05);两组支气管肺发育不良、视网膜病变发生率相比,无明显差异(P>0.05)。结论:传统、改良INSURE技术均能改善呼吸窘迫综合征早产儿血气指标,但改良INSURE技术还能降低患儿不良事件、并发症发生率。Objective:To investigate the clinical effect of the improved tracheal intubation-pulmonary surfactant-extubation(INSURE)technique in the treatment of respiratory distress syndrome of premature infants.Methods:Seventy-eight premature infants with respiratory distress syndrome admitted to Jingjiang People's Hospital from July 2022 to July 2024 were divided into two groups by random number table method,with thirty-nine cases in each group.The control group was treated with traditional INSURE technique,while the observation group was treated with improved INSURE technique.The therapeutic effect of the two groups was compared.Results:Before treatment,there were no significant differences in arterial partial pressure of oxygen,arterial partial pressure of carbon dioxide and pH between the two groups(P>0.05);after treatment,the arterial blood oxygen partial pressure and pH value were higher than before treatment,and the arterial blood carbon dioxide fraction was lower than before treatment,the difference was statistically significant(P<0.05);there was no significant difference between the two groups(P>0.05);the incidence of pulmonary surfactant reflux and transcutaneous oxygen saturation<85%in observation group was lower than that in control group(P<0.05);there were no significant differences in the incidence of bradycardia and apnea between the two groups(P>0.05);the incidence of air leakage syndrome and infection in observation group was lower than that in control group(P<0.05);there was no significant difference in the incidence of bronchopulmonary dysplasia and retinopathy between the two groups(P>0.05).Conclusion:Both traditional and improved INSURE technology can improve blood gas indexes of premature infants with respiratory distress syndrome,but the improved INSURE technology can also reduce the incidence of adverse events and complications.
