自制悬吊免气腹单孔腹腔镜手术在子宫肌瘤中的应用

摘　　要：目的探讨自制悬吊免气腹单孔腹腔镜手术在子宫肌瘤剔除术中的可行性及优势。方法回顾性选取2015年1月至2023年12月湘潭市第二人民医院妇产科收治的有手术指征的72例子宫肌瘤患者作为研究对象,根据手术方式不同分为免气腹组和传统组,每组36例。免气腹组采用自制悬吊免气腹腹腔镜下子宫肌瘤剔除术,传统组采用传统多孔充气腹腔镜下子宫肌瘤剔除术。收集两组患者围手术期各项资料,比较两组患者的手术时间、术中出血量、术后肠功能恢复时间和术后住院时间以及整体住院经济消耗。结果免气腹组的手术时间短于传统组,术中出血量少于传统组,术后肠道功能恢复时间短于传统组,差异有统计学意义(P<0.05)。两组患者的住院时间、住院经济消耗比较,差异无统计学意义(P>0.05)。结论相较于传统多孔充气腹腔镜手术,子宫肌瘤剔除术中采用自制悬吊免气腹腹腔镜手术缩短了手术时间,减少了术中出血,缩短了患者术后肠功能恢复时间,且该术式简单易行,无需增加额外设备及特殊材料,不增加医院投入及患者经济负担,经济实用,易于临床推广应用。Objective To explore the feasibility and advantages of self-made suspension non-pneumoperitoneum single-hole laparoscopic surgery in myomectomy.Methods A total of 72 patients with uterine fibroids with surgical indications admitted to the Department of Obstetrics and Gynecology of Xiangtan Second People's Hospital from January 2015 to December 2023 were retrospectively selected as the research objects,and they were divided into non-pneumoperitoneum group and traditional group according to different surgical methods,with 36 cases in each group.The non-pneumoperitoneum group was treated with self-made suspension non-pneumoperitoneum laparoscopic myomectomy,and the traditional group was treated with traditional porous pneumoperitoneum laparoscopic myomectomy.The perioperative data of the two groups were collected,and the operation time,intraoperative blood loss,postoperative intestinal function recovery time,postoperative hospitalization time and overall hospitalization economic cost were compared between the two groups.Results The operation time,intraoperative blood loss and postoperative intestinal function recovery time in non-pneumoperitoneum group were shorter than those in traditional group,and the differences were statistically significant(P<0.05).There were no significant differences in hospital stay and economic consumption between the two groups(P>0.05).Conclusion Compared with traditional porous pneumoperitoneum laparoscopic surgery,self-made suspension non-pneumoperitoneum laparoscopic myomectomy can shorten the operation time,reduce intraoperative bleeding,and shorten the postoperative intestinal function recovery time of patients.Moreover,this operation method is simple and easy,does not require additional equipment and special materials,does not increase hospital investment and patients'economic burden,and is economical and practical.It is easy for clinical application.
