血清学鉴定与全面生化反应在沙门菌检验中的价值

摘　　要：目的探讨血清学鉴定与全面生化反应在沙门菌检验中的价值。方法选取2021年1月至2022年12月湖北省荆州市第三人民医院收治的42例疑似肠热症患者作为研究对象,纳入研究者都需实施常规检验、血清学鉴定、全面生化反应,将复检结果作为检验金标准,比较常规检验、血清学鉴定联合全面生化反应检验结果诊断效能,同时对不同菌组对疾病的检验结果作进一步分析。结果42例疑似肠热症复查结果中检出沙门菌40例,2例未检出,阳性、阴性检出率分别为95.24%和4.76%;常规检验沙门菌检出33例,9例未检出,阳性、阴性检出率分别为78.57%和21.43%;血清学鉴定联合全面生化反应检验沙门菌检出39例,3例未检出,阳性、阴性检出率分别为92.86%和7.14%。常规检验灵敏度、特异度、准确度、阳性和阴性预测值分别为80.00%、50.00%、78.57%、11.11%、78.57%;血清学鉴定联合全面生化反应检验灵敏度、特异度、准确度、阳性和阴性预测值分别为97.50%、100.00%、97.62%、66.67%、97.62%;联合检测检验灵敏度、准确度以及阴性预测值高于常规检验,差异有统计学意义(P<0.05)。血清学鉴定联合全面生化反应检验在甲菌组、乙菌组的检验率高于常规检验,差异有统计学意义(P<0.05)。两种检验方式在丙菌组、丁菌组的检验率比较,差异无统计学意义(P>0.05)。结论临床检验沙门菌采用血清学鉴定与全面生化反应具有较高应用价值,与常规检验方法比较,检验准确性更高,可为临床检验沙门菌的后期标准制定提供有力的指导依据。Objective To explore the value of serological identification and comprehensive biochemical reaction in Salmonella detection.Methods A total of 42 patients with suspected enterofever treated in the Third People's Hospital of Jingzhou from January 2021 to December 2022 were selected as the research objects.All the included researchers were required to perform routine tests,serological identification and comprehensive biochemical reaction,and the retest results were taken as the gold standard.The diagnostic efficiency of routine test,serological identification combined with comprehensive biochemical reaction test were compared,and the results of different bacterial groups were further analyzed.Results Salmonella was detected in 40 cases and not detected in 2 cases.The positive and negative rates were 95.24%and 4.76%,respectively.Salmonella was detected in 33 cases and not detected in 9 cases.The positive and negative detection rates were 78.57%and 21.43%,respectively.Serological identification combined with comprehensive biochemical reaction test detected Salmonella in 39 cases,3 cases were not detected,the positive and negative detection rates were 92.86%and 7.14%,respectively.The sensitivity,specificity,accuracy,positive and negative predictive values of routine test were 80.00%,50.00%,78.57%,11.11%and 78.57%,respectively.The sensitivity,specificity,accuracy,positive and negative predictive values of serological identification combined with comprehensive biochemical reaction test were 97.50%,100.00%,97.62%,66.67%and 97.62%respectively.The sensitivity,accuracy and negative predictive value of combined test were higher than those of routine test,and the differences were statistically significant(P<0.05).The detection rates of serological identification combined with comprehensive biochemical reaction test in A group and B group were higher than those in routine test,and the differences were statistically significant(P<0.05).There was no significant difference between the two test methods in the test rate of C group
