摘 要:目的分析在系统性红斑狼疮抗双链DNA(dsDNA)抗体诊断中采取间接免疫荧光法(IIF)、酶联免疫吸附法(ELISA)、流式荧光法(FFIA)检验的临床价值。方法选取2021年1月至2022年12月枣庄市立医院收治的50例疑似系统性红斑狼疮患者作为研究对象,对所有患者均采取dsDNA抗体诊断,包括FFIA、IIF、ELISA检验,以免疫扩散作为dsDNA抗体检验金标准,分析不同检验方式下的结果,比较三种检验方式单独及联合检测的诊断价值。结果联合检验的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为90.0%、92.5%、92.0%、75.0%、97.4%,整体比较显示,联合检验的灵敏度、特异度、准确度、阳性预测值均高于单独FFIA、IIF、ELISA检测,差异有统计学意义(P<0.05)。结论FFIA、IIF、ELISA三种不同的检验方式在系统性红斑狼疮诊断中均有较高的诊断价值,联合诊断更能明显提高疾病诊断灵敏度与特异度,可作为临床实践中适用于系统性红斑狼疮诊断的抗dsDNA抗体诊断模式。Objective To analyze the clinical value of indirect immunofluorescence(IIF),enzyme-linked immunosorbent assay(ELISA)and flow fluorescence assay(FFIA)in the diagnosis of dsDNA antibodies in systemic lupus erythematosus.Methods Fifty patients with suspected systemic lupus erythematosus admitted to Zaozhuang Municipal Hospital from January 2021 to December 2022 were selected as the study objects.All patients were diagnosed with dsDNA antibody,included FFIA,IIF and ELISA tests.Immunodiffusion was used as the gold standard for dsDNA antibody test.The results under different test methods were analyzed,and the diagnostic value of the three tests alone and combined was compared.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 90.0%,92.5%,92.0%,75.0%and 97.4%,respectively.Overall comparison showed that the sensitivity,specificity,accuracy and positive predictive value of the combined test were higher than those of the single FFIA,IIF and ELISA tests,and the differences were statistically significant(P<0.05).Conclusion Three different test methods,FFIA,IIF and ELISA,have high diagnostic value in the diagnosis of systemic lupus erythematosus,and combined diagnosis can significantly improve the sensitivity and specificity of disease diagnosis,and can be used as an anti-dsDNA antibody diagnostic mode for the diagnosis of systemic lupus erythematosus in clinical practice.
