基于5E康复模式的延续性护理在维持性血液透析患者中的应用

摘　　要：目的探讨基于5E康复模式的延续性护理对维持性血液透析患者的干预效果。方法选取2023年3月至6月阜阳市人民医院收治的90例维持性血液透析患者作为研究对象,按照随机数字表法分为对照组(45例)和观察组(45例)。对照组给予常规护理,观察组在对照组的基础上给予基于5E康复模式的延续性护理方案。两组患者均护理12周,比较两组患者的自我管理能力[血液透析患者自我管理量表(SMSH)]、心理健康状况[焦虑自评量表(SAS)、抑郁自评量表(SDS)]及生活质量[简明健康状况调查量表(SF-36)]。结果干预后,两组患者SMSH、SF-36评分均高于本组干预前,且观察组SMSH、SF-36评分均高于对照组,差异有统计学意义(P<0.05)。干预后,两组患者SAS、SDS评分均低于本组干预前,且观察组SAS、SDS评分均低于对照组,差异有统计学意义(P<0.05)。结论基于5E康复模式的延续性护理干预能够显著提升维持性血液透析患者的自我管理能力,有效缓解其焦虑和抑郁情绪,进而提高其生活质量。Objective To explore the intervention effect of continuous care based on 5E rehabilitation model on maintenance hemodialysis patients.Methods A total of 90 patients with maintenance hemodialysis admitted to Fuyang People's Hospital from March to June 2023 were selected as the study objects and they were divided into control group(45 cases)and observation group(45 cases)according to random number table method.The control group was given routine nursing,and the observation group was given continuous care program based on 5E rehabilitation model.Both groups were treated for 12 weeks,and the self-management ability(self management scale for haemodialysis[SMSH]),mental health status(self-rating anxiety scale[SAS],self-rating depression scale[SDS])and quality of life(36-item short form health survey[SF-36])of patients in the two groups were compared.Results After intervention,SMSH and SF-36 scores in two groups were higher than those before intervention,and SMSH and SF-36 scores in observation group were higher than those of control group,the differences were statistically significant(P<0.05).After intervention,SAS and SDS scores of two groups were lower than those before intervention,and SAS and SDS scores of observation group were lower than those of control group,with statistical significances(P<0.05).Conclusion Continuous care intervention based on 5E rehabilitation model can significantly improve the self-management ability of maintenance hemodialysis patients,effectively relieve their anxiety and depression,and improve their quality of life.
