当代名中医运用炭类药治疗胎漏、胎动不安用药规律研究

摘　　要：目的:探索当代名中医临床运用炭类药治疗胎漏、胎动不安的用药规律。方法:从中国知网、万方、维普、读秀等数据库及图书馆筛选出名中医关于含炭类药治疗胎漏、胎动不安的处方,利用古今医案云平台(V2.3.7)构建数据库,进行数据挖掘。结果:共筛选出309首含炭类药治疗胎漏、胎动不安的处方,涉及中药246味,其中炭类药39味,高频炭类药为生地黄炭、黄芩炭、藕节炭等,多与桑寄生、续断、阿胶、白芍等补虚药配伍使用;处方中常用炭类药味数为1~3味;基于关联规则分析,得到含炭类药的核心处方:桑寄生、续断、阿胶、菟丝子、杜仲、苎麻根、白芍、白术、黄芩、黄芪、党参、墨旱莲、生地黄炭、当归、仙鹤草、甘草,并得到使用频次≥30的常用含炭类药物药对共26对;通过层次聚类分析得到含炭类药物新方核心组合5个。结论:当代名中医临床运用炭类药治疗胎漏、胎动不安时,常根据病情在处方中加入1~3味炭类药,如生地黄炭、黄芩炭、藕节炭等,配合补肾安胎、益气养血等中药,契合病机,体现了胎漏、胎动不安补肾固冲任、止血安胎之大法。Objective:To explore the medication rules of contemporary famous TCM doctors in the treatment of threatened abortion with carbonaceous drugs.Methods:The databases of CNKI,Wanfang,and VIP,and libraries were searched to screen the prescriptions of famous TCM doctors for the treatment of threatened abortion with carbonaceous drugs.The ancient and modern medical record cloud platform(v2.3.7)was used to build the database and carry out data mining.Results:A total of 309 prescriptions containing carbonaceous drugs for the treatment of threatened abortion were screened out,involving 246 traditional Chinese medicines,with 39 carbonaceous drugs.The high-frequency carbonaceous drugs were Shengdihuang(Radix Rehmanniae)carbonizing by stir-frying,Huangqin(Radix Scutellariae)carbonizing by stir-frying,Oujie(Nodus Nelumbins Rhizomatis)carbonizing by stir-frying,etc.,which were often used in combination with tonic drugs such as Sangjisheng(Herba Taxilli),Xuduan(Radix Dipsaci),Ejiao(Colla Corii Aaini),etc.The number of commonly used carbonaceous drugs in the prescription was 1 to 3.Based on the analysis of association rules,the core prescriptions of carbonaceous drugs were found to be Sangjisheng,Xuduan,Ejiao,etc.A total of 26 pairs of commonly used carbonaceous drug pairs with frequency not less than 30 times were obtained.Through hierarchical clustering analysis,five core combinations of carbonaceous drugs were obtained.Conclusion:When the contemporary famous TCM doctors clinically uses carbonaceous drugs to treat threatened abortion,they often add 1 to 3 carbonaceous drugs in to the prescription according to the condition,in order to cooperate with traditional Chinese medicines such as tonifying kidney,supplementing Qi and nourishing blood medicines.It embodies the therapeutic principle of tonifying kidney and consolidating Chong Ren(冲任),hemostasis and tocolysis in the treatment of threatened abortion.
