摘 要:糖尿病肾病是糖尿病常见并发症之一,为造成慢性肾脏病和终末期肾脏病的主要原因之一,同时也是造成糖尿病患者死亡的重要原因之一。转化生长因子(Transforming Growth Factor,TGF)-β1/Smad信号通路与糖尿病肾病的发病密切相关。TGF-β1是导致肾脏纤维化的关键因子,在与纤维化有关的慢性肾脏疾病中表达水平高,Smads蛋白是TGF-β1信号通路中下游的主要效应分子,TGF-β1激活Smad蛋白后将信号传送到细胞核中,并调节纤维化相关靶基因转录,从而促进肾脏纤维化进程。TGF-β1/Smad信号通路是肾脏纤维化的关键信号通路之一,而肾纤维化过程是慢性肾脏病的典型病理特征,因此干预肾脏纤维化有利于延缓糖尿病肾病进展为慢性肾功能衰竭。近年来,中医药在糖尿病肾病的防治中起到越来越重要的作用,关于中医药通过干预TGF-β1/Smad信号通路预防和治疗糖尿病肾病的相关研究也日益增多。本研究基于当下中医药治疗糖尿病肾病的研究现状,查阅相关文献,同时结合近几年来国内外相关研究,综述了TGF-β1/Smad信号通路的基本结构及其如何造成糖尿病肾病肾脏纤维化,并通过从中药中提取的有效成分中药单体干预TGF-β1/Smad信号通路,从而缓解炎症反应及氧化应激,减少细胞外基质沉积和上皮-间质转化等,起到抑制肾脏纤维化作用,延缓糖尿病肾病的进展,为中医药治疗糖尿病肾病的进一步研究提供新思路。Diabetic nephropathy is one of the common complications of diabetes,one of the main causes of chronic kidney disease and end-stage renal disease,and one of the important causes of death of diabetic patients.The transforming growth factor-β1(TGF-β1)/Smad signaling pathway is closely related to the pathogenesis of diabetic nephropathy.TGF-β1 is the key factor leading to renal fibrosis,and has a high expression level in patients with chronic kidney disease related to fibrosis.Smads protein is the main effector molecule in the middle and downstream of TGF-β1 signaling pathway.TGF-β1 activates Smad protein to transmit signals to the nucleus,and regulates the transcription of fibrosis-related target genes,thereby promoting the process of renal fibrosis.TGF-β1/Smad signaling pathway is one of the key signaling pathways in renal fibrosis,and the process of renal fibrosis is a typical pathological feature of chronic kidney disease.Therefore,intervention in renal fibrosis is conducive to delaying the progression of diabetic nephropathy to chronic kidney failure.In recent years,traditional Chinese medicine(TCM)has played an increasingly important role in the prevention and treatment of diabetic nephropathy,and there have been more and more studies on the prevention and treatment of diabetic nephropathy by intervening with TGF-β1/Smad signaling pathway.Based on the current research status of TCM in the treatment of diabetic nephropathy,consulting the relevant literature,combined with the relevant research at home and abroad in recent years,the basic structure of TGF-β1/Smad signaling pathway and its mechanism of renal fibrosis in diabetic nephropathy is summarized in this paper.In addition,TGF-β1/Smad signaling pathway is intervened with TCM monomer,the active ingredient extracted from TCM medicine,thereby alleviating inflammatory response and oxidative stress,reducing extracellular matrix deposition and epithelial-interstitial transformation,and inhibiting renal fibrosis,so as to delay the progression of diabetic n
