健脾祛湿中药对高尿酸血症患者肠道菌群作用机制的研究进展

摘　　要：高尿酸血症是一种内分泌系统疾病,多由嘌呤代谢障碍所引起。部分高尿酸血症患者最终会进展为痛风或其他慢性症,严重影响患者的生活质量,且呈现逐渐流行的趋势,业已引起临床高度重视,同时也成为诸多医家热衷的课题。高尿酸血症与尿酸代谢密切相关,西医将促进尿酸排泄以及抑制尿酸合成等方法作为常用的治疗手段,然而西医药物治疗易发生消化道损伤、肝肾损伤等不良反应。动物实验研究和临床研究均表明高尿酸血症的发生发展与机体肠道菌群及其代谢产物改变有密切的联系,中药及其复方可以通过调控肠道菌群和相关代谢途径改善高尿酸血症。中医认为脾为后天之本,高尿酸血症多是由于脾虚湿生引起的,因此治疗上注重健脾祛湿类中药的运用。中药可以影响宿主肠道菌群的结构和功能,发挥降尿酸作用,菌群可以将中药活性物质代谢分解为活性更强的物质从而发挥药效。由于中医药在治疗上具有多成分、多途径、综合调控等特点,可调控肠道菌群以达到降尿酸效果,能够为患者提供更多治疗手段选择,并降低药物不良反应风险。文章旨在总结健脾祛湿中药对高尿酸血症患者肠道菌群的作用机制,以期为临床提供参考。Hyperuricemia is an endocrine system disease,which is usually caused by purine metabolism disorders.Some patients with hyperuricemia will eventually become gout or other chronic diseases,which will seriously affect the patient’s quality of life.This disease presents an increasing epidemic trend year by year,which has attracted great attention in clinical practice,and has also become a subject that many experts are keen to study.This disease is closely related to uric acid metabolism.Western medicine aways promotes the excretion of uric acid and inhibit the synthesis of uric acid,as common treatment methods.However,Western medicine is easy to cause gastrointestinal injury,liver and kidney injury and other adverse reactions.Animal experiments and clinical studies have shown that the occurrence and development of hyperuricemia are closely related to the changes of intestinal flora and its metabolites.Traditional Chinese medicine and its compound can improve hyperuricemia by regulating intestinal flora and related metabolic pathways.Traditional Chinese medicine believes that the spleen is the acquired foundation,and hyperuricemia is mostly caused by spleen deficiency and dampness.Therefore,the use of traditional Chinese medicine for invigorating the spleen and removing dampness is emphasized in the treatment.Traditional Chinese medicine can affect the structure and function of the host intestinal flora to play the role of reducing uric acid,and the flora can decompose the active substances of traditional Chinese medicine into more active substances to strengthen the efficacy.Traditional Chinese medicine has the characteristics of multi-component,multi-channel and comprehensive regulation in the treatment,and can regulate intestinal flora to achieve the effect of reducing uric acid,providing more treatment options for patients and reducing adverse reactions.This paper aims to summarize the mechanism of spleen invigorating and dampness removing traditional Chinese medicine on intestinal flora in patients with hyperur
