基于慢性肾脏病的中医体质影响因素研究现状

摘　　要：文章通过梳理慢性肾脏病(Chronic Kidney Disease,CKD)的中医体质影响因素研究探究CKD人群不同体质类型的形成因素,从中医体质生理因素、先后天因素进行分析。目前,相关研究中CKD中医体质形成的先天因素主要包括家族史、基因、血型、过敏等,后天因素主要包括饮食嗜好、生活起居、精神情志等,生理因素主要包括体质量指数、形态结构、年龄、性别方面。研究结果显示,CKD的中医体质形成与家族史、基因、过敏等影响因素有关,但与血型、体质量指数、年龄等影响因素间的联系存在争议。其中,跟气虚质有关的因素有过敏、体质量指数、饮食等;跟阳虚质有关的因素有低体质量、饮食、过敏等;跟阴虚质有关的因素有饮食、睡眠障碍、外邪;跟痰湿质有关的因素有超重/肥胖、饮食、过敏等;跟湿热质有关的因素有超重/肥胖、短病程、岭南地区等;跟血瘀质有关的因素有睡眠障碍、长病程、有家族史的早期糖尿病肾病患者;跟气郁质有关的因素有睡眠质量、情志;跟特禀质有关的因素有过敏、饮食。文章最后提出中医体质影响因素研究面临的问题,以期为体质流行病学调查、中医病因理论研究提供参考。Through combing the research on the influencing factors of traditional Chinese medicine(TCM)constitution in chronic kidney disease(CKD),this article explores the formation factors of different constitution types in patients with CKD,and analyzes the physiological factors,innate factors and acquired factors of TCM constitution.At present,the innate factors of the formation of TCM constitution in CKD are mainly family history,gene,blood groups and allergic factors.The acquired factors are mainly diet habits,daily life,spiritual emotions,etc..The physiological factors are mainly body mass index,somatotype of morphological structure,age and gender.It is concluded that the formation of TCM constitution in CKD is related to influencing factors such as family history,gene,and allergy,but it is controversial with influencing factors such as blood groups,body mass index,age,etc..Among them,Qi(气)deficiency constitution is related to factors such as allergy,body mass index,diet,etc..Yang(阳)deficiency constitution is related to factors such as low body weight,diet,allergy,etc..Yin(阴)deficiency constitution is related to factors such as diet,sleep disorders,external pathogen.Phlegm dampness constitution is related to factors such as overweight/obesity,diet,allergy,etc..Damp heat constitution is related to factors such as overweight/obesity,short course of disease,and region of Lingnan(岭南)area.Blood stasis constitution is related to factors such as sleep disorders,long course of disease,and early diabetic nephropathy in patients with family history.Qi stagnation constitution is related to factors such as sleep quality and emotion.Special-diathesis constitution is related to factors such as allergy and diet.Finally,the problems faced by the research on the influencing factors of TCM constitution are put forward,to provide reference for the epidemiological investigation of constitution and the theoretical research of TCM etiology.
