广东省内科住培结业临床实践能力统考模式的探索

摘　　要：目的分析广东省内科住院医师规范化培训(简称住培)结业临床实践能力统考的实践,通过总结经验推进全面客观及同质化的住培医师能力评估。方法2022年5月—2023年6月采取客观结构化临床考核(objective stractured clinical examination,OSCE)方式,2022年开始统一组织实施广东省内科住培结业临床实践能力考核,并对2022年和2023年考核实施的具体情况及考生、考官的问卷调查结果进行综合分析。2022年共381名考生和168名考官完成调查问卷,2023有930名考生和323名考官完成调查问卷。结果2022年50.39%和2023年45.38%的考生认为考核组织形式比较好,2022年32.81%和2023年29.35%的考生认为考核组织形式非常好,对比2年的评价,差异有统计学意义(P<0.05)。2022年及2023年考生认为第一、四和五考站的考核难度适中。而考生普遍认为第二及第三考站的考核难度比较难或非常难,对比难度评价,差异有统计学意义(P<0.05)。对比2022年及2023年考生对第一考站考核形式及内容的评价,差异无统计学意义(P>0.05);而对第二、三、四和五考站考核形式及内容的评价,差异有统计学意义(P<0.05)。2022年60.12%和2023年58.52%的考官认为考核形式及内容比较好,2022年22.62%和2023年31.44%的考生认为考核形式及内容非常好,差异有统计学意义(P<0.05)。2022年55.95%和2023年44.33%的考官认为考核组织情况比较好,2022年38.69%和2023年50.52%的考生认为考核组织情况非常好,差异有统计学意义(P<0.05)。结论内科住培结业临床实践能力统一考核能更客观地评估全省各个内科基地的培训质量,促进内科医师规范化、同质化培养。Objective To analyze the practice of unified examination of clinical practice ability after standardized training of internal medicine residents in Guangdong Province,and to promote comprehensive,objective and homogeneous evaluation of residential training physicians’ability.Methods From May 2022 to June 2023,objective stractured clinical examination(OSCE)was adopted to evaluate the clinical practice ability of internal medicine students in Guangdong province from 2022.The specific situation of the implementation of the examination in 2022 and 2023 and the results of questionnaire survey were analyzed.In 2022,381 candidates and 168 examiners completed the questionnaire,and in 2023,930 candidates and 323 examiners completed the questionnaire.Results There was 50.39%candidates in 2022 and 45.38%in 2023 thought that the assessment organization form was good,32.81%in 2022 and 29.35%in 2023 thought that the assessment organization form was very good,compared the two years,and the differences were statistically significant(P<0.05).The candidates in 2022 and 2023 thought the first,Thursday and five test stations were moderately difficult.The test takers generally thought that the second and third test stations were more difficult or very difficult,and the differences were statistically significant(P<0.05).In 2022 and 2023,there was no statistical difference in the evaluation of the form and content of the first test station(P>0.05),but in the evaluation of the form and content of the second,third,fourth and fifth test stations,and the differences were statistically significant(P<0.05).There was 60.12%examiners in 2022 and 58.52%in 2023 thought that the form and content of the examination were good,22.62%in 2022 and 31.44%in 2023 thought that the form and content of the examination were very good,and the differences were statistically significant(P<0.05).There was 55.95%examiners in 2022 and 44.33%in 2023 thought that the assessment organization was relatively good,38.69%in 2022 and 50.52%in 2023 thought that the asses
