WAFF运动与盆底肌筋膜手法联合电刺激生物反馈对经阴道分娩轻度阴道松弛症患者盆底功能的影响

摘　　要：目的比较WAFF运动与盆底肌筋膜手法联合电刺激生物反馈对经阴道分娩轻度阴道松弛症患者盆底功能的影响。方法选取2021年7月至2022年11月收治的112例经阴道分娩的轻度阴道松弛症患者,根据不同治疗方案分为WAFF运动联合电刺激生物反馈组(观察组A)、盆底肌筋膜手法联合电刺激生物反馈组(观察组B)、单纯电刺激生物反馈组(对照组)。比较3组患者治疗前后女性性功能指数调查量表(FSFI)评分、盆底表面肌电检测结果、阴道中段前后壁厚度的差异。结果3组患者治疗后FSFI各维度评分及总分值相较于治疗前均显著上升(P<0.05)。治疗后观察组A患者性欲分值、性唤起分值、总分值及其差值均高于观察组B和对照组(P<0.05),治疗后观察组B性交痛分值及其差值均高于观察组A、对照组(P<0.05)。3组患者治疗后盆底表面肌电检测前静息阶段平均值、后静息阶段平均值相较于治疗前均显著下降,快速收缩阶段最大值、紧张收缩阶段平均值、耐力收缩阶段平均值相较于治疗前均显著上升(P<0.05)。治疗后观察组B患者前静息阶段平均值、后静息阶段平均值低于观察组A和对照组,及其差值均高于观察组A和对照组(P<0.05);治疗后观察组A患者快速收缩阶段最大值、紧张收缩阶段平均值、耐力收缩阶段平均值及其差值均高于观察组B和对照组(P<0.05)。3组患者治疗后超声测量阴道中段前壁及后壁厚度分别较治疗前显著上升(P<0.05)。治疗后观察组B患者超声测量阴道中段前壁及阴道中段后壁厚度、差值均高于观察组A和对照组(P<0.05)。结论WAFF运动与盆底肌筋膜手法联合电刺激生物反馈治疗轻度阴道松弛症均具有良好效果,前者可提高性欲、性唤起、性生活总体质量和提升盆底肌力、肌肉协调性,后者可降低盆底张力、缓解盆底肌痉挛、改善性交不适感、增加阴道壁厚度,临床可根据患者实Objective To compare the effects of WAFF exercise and pelvic floor myofascia manipulation on pelvic floor function combired with electrical sitmulation biofeedback in patients with mild vaginal relaxation during vaginal delivery.Methods A total of 112 patients with mild vaginalysis who delivered vaginalysis from July 2021 to November 2022 were se⁃lected and divided into WAFF exercise combined with electrical stimulation biofeedback group(observation group A),pelvic floor myofascial manipulation combined with electrical stimulation biofeedback group(observation group B)and pure electrical stimulation biofeedback group(control group)according to different treatment plans.The differences of female Sexual Func⁃tion Index(FSFI)scores,pelvic floor surface electromyography(EMG)test results,and vaginal midsection wall thickness were compared among 3 groups.Results After treatment,the score and total score of FSFI in all three groups were signifi⁃cantly increased compared with before treatment(P<0.05).After treatment,the sexual desire score,sexual arousal score,total score and their differences in observation group A were higher than those in observation group B and control group(P<0.05),and the coitus pain score and their differences in observation group B were higher than those in observation group A and control group(P<0.05).Compared with before treatment,the mean value of resting stage and the mean value of post⁃resting stage of pelvic floor muscle surface EMG detection in 3 groups were significantly decreased after treatment,and the maximum value of rapid contraction stage,the mean value of tension contraction stage and the mean value of endurance con⁃traction stage were significantly increased compared with before treatment(P<0.05).After treatment,the average values of the pre⁃resting stage and post⁃resting stage in observation group B were lower than those in observation group A and control group,and their differences were higher than those in observation group A and control group(P<0.05).After treatm
