摘 要:目的将乙酰半胱氨酸与阿奇霉素序贯疗法联合用于小儿支原体肺炎患儿治疗中,探究其临床效果及对炎症因子、血清可溶性程序性死亡受体1(sPD-1)、细胞因子样蛋白1(CYTL1)水平的影响。方法回顾性取2020年9月—2022年8月本院收治的88例小儿支原体肺炎患儿,根据治疗方案将其分为两组,每组44例。两组均给予常规治疗,对照组在此基础上采用阿奇霉素序贯疗法治疗,观察组在对照组基础上加用乙酰半胱氨酸治疗,评价两组治疗效果。结果观察组治疗总有效率为95.45%,较对照组81.82%高(P<0.05);两组治疗前的白细胞介素10(IL-10)及C反应蛋白(CRP)、降钙素原(PCT)水平相比,差异无统计学意义(P>0.05);治疗后,观察组各指标水平较对照组更低(P<0.05);治疗前,组间sPD-1、CYTL1水平相比,差异无统计学意义(P>0.05);治疗后,观察组sPD-1水平更低、CYTL1水平更高(P<0.05)。结论对小儿支原体肺炎患儿而言,应用乙酰半胱氨酸与阿奇霉素序贯疗法相结合的治疗方案具有满意的临床效果,既能减轻患儿炎症反应,还可调节sPD-1、CYTL1表达,值得推广。Objective To investigate clinical effect of acetylcysteine and azithromycin sequential therapy in treatment of children with mycoplasma pneumonia,and its effect on inflammatory factors,sPD-1,and CYTL1 levels.Methods The paper reviewed and chose 88 children with mycoplasma pneumonia in our hospital from September 2020 to August 2022,and divided them into two groups according to different treatment plans,treated with routine treatment,while the control group(44 cases)with azithromycin sequential therapy on the basis,the observation group with acetylcysteine on the basis of the control group.Curative effect between two groups was evaluated.Results Total effective rate of the observation group was 95.45%,higher than 81.82%in the control group,(P<0.05).There was no significant difference in levels of interleukin-10(IL-10),C-reactive protein(CRP),and procalcitonin(PCT)between two groups before treatment(P>0.05).After treatment,levels of various indicators in the observation group were lower(P<0.05).Before treatment,there was no significant difference in levels of sPD-1 and CYTL1 between the groups(P>0.05).After treatment,the observation group had lower levels of sPD-1 and higher levels of CYTL1(P<0.05).Conclusion Combination of acetylcysteine and azithromycin sequential therapy can achieve satisfactory clinical effect for children with mycoplasma pneumonia,can reduce inflammatory response,regulate the expression of sPD-1 and CYTL1,which is worthy of promotion.
