肌骨超声检测无症状高尿酸血症的临床应用效果分析

摘　　要：目的:探讨肌骨超声检测无症状高尿酸血症(HUA)的临床应用价值。方法:选取2022年2月—2024年2月航天中心医院收治的42例无症状HUA患者和45例痛风性关节炎(GA)患者分别纳入无症状HUA组和痛风组,另选取同期50名健康体检者纳入健康组,均接受肌骨超声检查,分析三组生化指标、肌骨超声征象和关节受累情况。结果:无症状HUA组和痛风组血尿酸、空腹血糖、收缩压和舒张压水平均高于健康组,差异均有统计学意义(P<0.05);无症状HUA组痛风石与各征象总阳性检出率高于健康组,痛风组双轨征、滑膜增生、骨侵蚀与各征象总阳性检出率高于无症状HUA组,无症状HUA组膝关节受累检出率低于痛风组,差异均有统计学意义(P<0.05)。结论:肌骨超声检测无症状HUA患者关节阳性率较高,可敏感显示其早期关节病变特征及关节受累情况。Objective To explore the clinical application value of musculoskeletal ultrasound for detecting asymptomatic hyperuricaemia(HUA).Methods From February 2022 to February 2024,42 patients with asymptomatic HUA and 45 patients with gouty arthritis(GA)admitted to the Aerospace Center Hospital were selected to be included in the asymptomatic HUA group and the gout group,respectively,and 50 healthy individuals who underwent physical examinations during the same period were selected to be included in the healthy group.All of them received musculoskeletal color ultrasound examination,and the biochemical indexes,musculoskeletal ultrasound signs and joint involvement of the three groups were analyzed.Results Blood uric acid,fasting blood glucose,systolic blood pressure and diastolic blood pressure levels were higher in both the asymptomatic HUA group and the gout group than that in the healthy group(P<0.05);the detection rate of gout stones and total positive detection rate of each sign was higher in the asymptomatic HUA group than that in the healthy group,and the detection rate of double-tracking sign,synovial hyperplasia,bone erosion and total positive detection rate of each sign in the gout group was higher than those in the asymptomatic HUA group,the detection rate of knee joint involvement in the asymptomatic HUA group was lower than that in the gout group(P<0.05).Conclusion The positive rate of musculoskeletal ultrasound joints in patients with asymptomatic HUA is higher,which can sensitively show the characteristics of early arthropathy and joint involvement.
