摘 要:目的建立高血压强化定量运动管理技术,探讨强化定量运动与高血压的关系,为制定高血压的干预措施提供技术支持。方法采用实验流行病学研究方法,于2021年6月,采取整群随机抽样方式抽取苏州工业园区4个社区,并将其以简单随机抽样的方式分为实验组、对照组社区各2个。采用系统抽样方法从实验组社区和对照组社区各抽取181名高血压患者,分别进行一年的强化定量运动干预及传统社区高血压管理,分析两组的血压、血脂及心率等指标,比较两组的干预效果。结果干预后,干预组、对照组血压控制优良率分别为61.33%(111/181)、46.41%(84/181),干预组人群血压控制优良率高于对照组(χ^(2)=8.104,P=0.004),且收缩压、舒张压下降水平高于对照组(t值分别为3.654、3.516,P<0.001)。干预后,干预组低密度脂蛋白胆固醇降低0.18 mmol/L,对照组高密度脂蛋白胆固醇水平下降0.07 mmol/L(t值分别为3.561、5.979,P<0.001),但干预组、对照组心率均较干预前下降(t值分别为5.405、2.485,P<0.001)。调整混杂因素后,运动干预、心率均与高血压存在关联,RR分别0.498(95%CI:0.318~0.782)、1.036(95%CI:1.010~1.061),P<0.05。结论强化定量运动能够改善高血压人群的血压、血脂等指标,提高高血压控制率。Objective To establish the management technology of enhanced quantitative exercise for hypertension to explore the relationship between enhanced quantitative exercise and hypertension, so as to provide technical support of the intervention measures for hypertension. Methods Experimental epidemiological research methods were used in this study.In June, 2021,4 communities in Suzhou Industrial Park were selected by cluster random sampling, and they were randomly divided into experimental group and control group with 2 communities each by simple random sampling.A systematic sampling method was used to select 181 hypertensive patients from each of the experimental and control communities for 1 year of intensive quantitative exercise intervention and traditional community-based hypertension management, respectively, to analyze the indicators of blood pressure, blood lipids, and heart rate of the 2 groups, and to compare the effects of the interventions in the 2 groups. Results After intervention, hypertension under control in the intervention group and the control group were 61.33%(111/181) and 46.41%(84/181) respectively, and hypertension under control in the intervention group was higher than that in the control group(χ^(2)=8.104,P=0.004).Compared with the control group, the systolic and diastolic blood pressure in the intervention group decreased more obviously(t=3.654,3.516,P<0.001).After intervention, the low density lipoprotein cholesterol in the intervention group decreased by 0.18 mmol/L,and the high density lipoprotein cholesterol in the control group decreased by 0.07mmol/L(t= 3.561,5.979,P<0.001),but the heart rate of the intervention group and the control group decreased compared with that before intervention(t=5.405,2.485,P<0.001).After adjusting for confounding factors, exercise intervention and heart rate were obviously related to hypertension, RR were 0.498(95%CI:0.318-0.782) and 1.036(95%CI:1.010-1.061) respectively, P<0.05.
