摘　　要：目的探究主观垂直(水平)视觉(subjective vision vertical/horizontal,SVV/SVH)常值及其应用于发作期良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)患者耳石器功能评价的临床价值。方法纳入2023年9月至2023年12月在中国人民解放军总医院就诊的诊断为原发性BPPV患者60例为试验组,2023年9月至2024年5月中国人民解放军总医院、宁夏回族自治区人民医院无眩晕健康志愿者30例为对照组。试验组依据患侧和年龄,分别进行SVV/SVH头正中位(0°)、头右倾45°、头右倾75°、头左倾45°、头左倾75°测试,比较不同人群及同侧别BPPV患者不同年龄组的SVV/SVH均值与方差。结果BPPV患者SVV向病变侧倾斜,SVH右侧BPPV组顺时针倾斜(呈正值)、左侧BPPV组逆时针倾斜(呈负值);SVH较SVV表现出更大的组间差异;年龄较大BPPV患者在患侧偏转时较易出现更大方差,且75°的偏斜角度较常规45°更易出现异常。结论对照组SVV/SVH在不同测试头位的偏斜角度存在差异,各实验室应建立正常值参考范围。SVV/SVH测试对于BPPV患者病变侧别有提示意义,方差是SVV/SVH检查的重要参数,SVV/SVH检查对于耳石器官功能评价有一定的临床意义,有望加入常规前庭功能检查序列中对耳石器官功能进行评估。Objective To report normal values of subjective visual vertical/horizontal(SVV/SVH)and its value in clinical evaluation of otolith function in patients with paroxysmal benign paroxysmal positional vertigo(BPPV).Methods Patients diagnosed with primary BPPV from September 2023 to December 2023 at Chinese PLA General Hospital(n=60)were included as the study group,and 30 healthy volunteers without vertigo of Chinese PLA General Hospital and People's Hospital of Ningxia Hui Autonomous Region were recruited from September 2023 to May 2024 as the control group.Patients were grouped based on affected side and age.SVV and SVH were tested at head median(0°),right 45°,right 75°,left 45°and left 75°.SVV,SVH mean and variance were compared among groups.Results At the onset of BPPV,SVV tilted to the lesion side.SVH tilted clockwise(positive)in right side BPPV and counterclockwise(negative)in left side BPPV.SVH showed greater between-group difference than SVV.Older patients were more likely to show greater variance when tilting to the affected side,more so at 75°than at 45°.Conclusion SVV and SVH deviation angles can vary in the healthy individuals at different head positions,and each laboratory should establish their own normal value range.SVV and SVH tests can help locate the lesion side in BPPV.Variance is an important parameter in SVV,SVH examination.SVV,SVH have certain clinical significance for evaluation of otolith organ function,and is expected to be added to the routine vestibular function examination sequence.
