普通针刺结合朱氏头皮针透刺治疗周围性面瘫临床观察

摘　　要：目的观察普通针刺结合朱氏头皮针透刺治疗周围性面瘫的临床疗效。方法将60名患者按随机数字表法分为治疗组与对照组,每组30例。两组均采用健康宣传教育及药物基础治疗,对照组予以常规针刺治疗,每次30 min,隔日1次;治疗组在对照组基础上加用朱氏头皮针透刺治疗,每次头针留针6 h,隔日1次,取穴及方法同对照组。治疗前及治疗4周后采用House-Brackmann(H-B)面神经功能分级量表,面部残疾指数(facial disability idex,FDI)量表,健、患侧红外热成像温差,徒手肌力分级检查评价面部功能恢复情况,基于治疗前后H-B分级变化判定临床疗效。结果在降低眶上区和口周区健、患侧红外热成像温差,FDI社会维度评分,改善眼睑肌力和口周肌力,提高FDI生理维度评分方面,治疗组显著优于对照组(P<0.05);治疗组基于H-B分级的临床疗效显著优于对照组(P<0.05)。结论在改善周围性面瘫患者症状,提高眼睑肌力、口周肌力方面,普通针刺结合朱氏头皮针透刺法疗效优于普通针刺法。Objective To investigate the clinical effect of conventional acupuncture combined with Zhu's scalp acupuncture in the treatment of peripheral facial paralysis.Methods A total of 60 patients were divided into treatment group and control group using a random number table,with 30 patients in each group.The patients in the control group were given health education,basic drug treatment,and conventional acupuncture for 30 minutes each time,once every other day,and those in the treatment group were given Zhu's scalp acupuncture in addition to the treatment in the control group,with needle retaining for 6 hours,once every other day,using the same methods for acupoint selection and treatment as the control group.Before treatment and after 4 weeks of treatment,the House-Brackmann(H-B)facial nerve grading system,Facial Disability Index(FDI)scale,temperature difference between the unaffected and affected sides measured by infrared thermal imaging,and manual muscle testing were used to assess the recovery of facial function,and clinical outcome was determined based on the change in H-B grade after treatment.Results Compared with the control group,the treatment group had significantly greater reductions in temperature differences of the supraorbital and perioral regions between the unaffected and affected sides measured by infrared thermal imaging,a significantly greater reduction in the score of the social dimension of FDI,significantly greater improvements in eyelid muscle strength and perioral muscle strength,and a significantly greater increase in the physiological dimension of FDI(P<0.05).The treatment group had a significantly better clinical outcome than the control group based on H-B grade(P<0.05).Conclusion Conventional acupuncture combined with Zhu's scalp acupuncture has a better clinical effect than conventional acupuncture alone in improving the symptoms of patients with peripheral facial paralysis and increasing eyelid muscle strength and perioral muscle strength.
