动气针法针刺灵骨、大白穴治疗脑卒中后手功能障碍临床观察

摘　　要：目的观察动气针法针刺灵骨、大白穴对脑卒中后手功能障碍的临床疗效。方法将60例脑卒中后手功能障碍患者随机分为观察组和对照组,每组30例。两组均接受常规药物及康复治疗,对照组给予常规头针、体针治疗,观察组在对照组治疗基础上,予以动气针法针刺健侧灵骨、大白穴,两组均治疗4周。采用Fugl-Meyer评定量表(Fugl-Meyer assessment,FMA)手部运动功能评分、手功能评定量表评分、改良Barthel指数(modified Barthel index,MBI)评分及Brunnstrom分级评价手部运动功能和日常生活能力。结果与治疗前比较,治疗后两组患者FMA手部运动功能评分、手功能评定量表评分、MBI评分、Brunnstrom分级均显著升高(P<0.05),且观察组上述指标升高程度显著大于对照组(P<0.05)。结论在常规针刺和头皮针的治疗基础上,采用动气针法针刺灵骨、大白穴可有效改善脑卒中患者手部运动功能,提高其日常生活能力。Objective To investigate the clinical effect of activating Qi acupuncture at Linggu and Dabai points in the treatment of hand dysfunction after stroke.Methods A total of 60 patients with hand dysfunction after stroke were randomly divided into observation group and control group,with 30 patients in each group.The patients in the control group were given routine medication,rehabilitation treatment,scalp acupuncture,and body acupuncture,and those in the observation group were given activating Qi acupuncture at contralateral Linggu and Dabai points in addition to the treatment in the control group.The course of treatment was 4 weeks for both groups.Fugl-Meyer Assessment(FMA)hand score,hand function rating scale score,modified Barthel index(MBI)score,and Brunnstrom stage were used to assess hand motor function and activities of daily living.Results After treatment,both groups had significant increases in FMA hand score,hand function rating scale score,MBI score,and Brunnstrom stage(P<0.05),and the observation group had significantly greater increases than the control group(P<0.05).Conclusion In addition to conventional acupuncture and scalp acupuncture,activating Qi acupuncture at Linggu and Dabai points can effectively improve hand motor function and enhance activities of daily living in patients with stroke.
