摘 要:目的观察李氏定位旋转复位法对神经根型颈椎病(cervical spondylotic radiculopathy,CSR)患者颈椎曲度的影响。方法将78例CSR患者随机分为观察组和对照组,对照组予以颈椎牵引及常规推拿治疗,观察组在颈椎牵引及常规推拿治疗基础上加用李氏定位旋转复位法。观察两组治疗后临床疗效,治疗前后分别测量C_(2-7) Cobb角度数、Borden值、颈椎活动度,采用田中靖久颈椎病症状量表和颈椎功能障碍指数(neck disability index,NDI)量表评价颈椎的功能状态。结果观察组临床疗效显著优于对照组(P<0.05)。在提高C_(2-7) Cobb角度数,Borden值,颈椎前屈、后伸、左旋、右旋角度,田中靖久颈椎病症状量表评分,以及降低NDI量表评分方面,观察组均显著优于对照组(P<0.05)。结论在颈椎牵引及常规推拿治疗基础上,李氏定位旋转复位法能够更好地改善CSR患者的颈椎曲度,减轻伴随症状。Objective To investigate the effect of Li’s positioning and rotary reduction technique on cervical curvature in patients with cervical spondylotic radiculopathy(CSR).Methods A total of 78 patients with CSR were randomly divided into observation group and control group.The patients in the control group were given cervical traction and conventional massage,while those in the observation group were given Li’s positioning and rotary reduction in addition to the treatment in the control group.Clinical outcome was observed for both groups after treatment;C_(2-7) Cobb angle,Borden value,and cervical range of motion(CROM)were measured before and after treatment,and Yasuhisa Tanaka’s Cervical Spondylosis Symptom Scale and Neck Dysfunction Index(NDI)were used to evaluate cervical function.Results The observation group had a significantly better clinical outcome than the control group(P<0.05).Compared with the control group,the observation group had significantly greater increases in C_(2-7) Cobb angle,Borden value,cervical flexion/extension/left rotation/right rotation angles,and Yasuhisa Tanaka’s Cervical Spondylosis Symptom Scale score and a significantly greater reduction in NDI score(P<0.05).Conclusion In addition to cervical traction and conventional massage,Li’s positioning and rotary reduction technique can better improve cervical curvature and alleviate accompanying symptoms in patients with CSR.
