摘  要:目的分析妊娠期糖尿病(GDM)患者孕中期的生长分化因子-15(GDF15)mRNA表达与糖脂代谢指标的相关性。方法选择2022年7月—2023年12月在曲靖市第二人民医院产检的100例孕中期(孕周22~28周)GDM患者纳入GDM组;另外选择100例孕中期健康孕妇纳入对照组。采用化学法和荧光定量PCR法检测所有受检者糖脂代谢指标〔空腹血糖(FBG)、餐后1 h血糖(1hPG)、餐后2 h血糖(2hPG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)〕以及GDF15 mRNA,比较两组上述指标水平差异。采用Spearman相关性分析方法考察GDF15 mRNA与FBG、1hPG、2hPG、TC、TG、HDL-C、LDL-C的相关性;采用多元Logistic回归分析考察GDF15 mRNA对糖脂代谢指标异常的影响。结果GDM组FBG、1hPG、2hPG、TC、TG和HDL-C、GDF15 mRNA水平均显著高于对照组〔FBG(mmol/L):5.12±0.43比4.53±0.35;1hPG(mmol/L):9.33±1.67比7.14±1.32;2hPG(mmol/L):8.19±1.57比6.01±1.12;TC(mmol/L):4.50±1.31比4.15±1.14;TG(mmol/L):2.42±0.95比1.48±0.52;HDL-C(mmol/L):1.27±0.44比1.14±0.37;GDF15 mRNA(拷贝/μL):6348.00(2547.00,13035.00)比72.97(23.14,151.50);均P<0.05〕。GDM组血清GDF15 mRNA与FBG、1hPG、2hPG、TC、TG、HDL-C均呈正相关(r值分别为0.571、0.552、0.590、0.151、0.521、0.193,P值分别为<0.001、<0.001、<0.001、0.037、<0.001、0.006)。多元Logistic回归分析显示以对照组为参照,随着GDF15 mRNA水平升高,出现糖脂代谢指标1项、2项、多项异常的风险逐渐升高〔优势比(OR)分别为2.03、5.47、18.23,P值分别为0.537、0.049、<0.001〕。结论血清GDF15 mRNA表达越高,孕妇出现糖脂代谢异常的风险越高,程度越重,该指标检测可以有效预测妊娠期糖脂代谢异常。Objective To analyze the correlation between the expression of growth differentiation factor-15(GDF15)mRNA and glucose and lipid metabolism indexes in pregnant women with gestational diabetes mellitus(GDM)in the second trimester.Methods The 100 patients in mid pregnancy(22-28 weeks)with GDM who underwent prenatal check-ups at the Second People's Hospital of Qujing from July,2022 to December,2023 were selected as research subjects and included in GDM group.Additionally,100 healthy pregnant women in mid pregnancy were selected as control group.The glucose and lipid metabolism indicators[fasting blood glucose(FBG),1-hour postprandial blood glucose(1hPG),2-hour postprandial blood glucose(2hPG),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C)]and GDF15 mRNA were detected in all subjects using chemical method and fluorescent quantitative PCR method.The differences in the levels of above indicators between two groups were compared.The correlation between GDF15 mRNA and FBG,1hPG,2hPG,TC,TG,HDL-C,LDL-C was investigated using Spearman correlation analysis.Results The levels of FBG,1hPG,2hPG,TC,TG,HDL-C and GDF15 mRNA in GDM group were higher than those in control group[FBG(mmol/L):5.12±0.43 vs.4.53±0.35;1hPG(mmol/L):9.33±1.67 vs.7.14±1.32;2hPG(mmol/L):8.19±1.57 vs.6.01±1.12;TC(mmol/L):4.50±1.31 vs.4.15±1.14;TG(mmol/L):2.42±0.95 vs.1.48±0.52;HDL-C(mmol/L):1.27±0.44 vs.1.14±0.37;GDF15 mRNA(copies/μL):6348.00(2547.00,13035.00)vs.72.97(23.14,151.50);all P<0.05].The serum GDF15 mRNA in GDM group was positively correlated with FBG,1hPG,2hPG,TC,TG and HDL-C(r values were 0.571,0.552,0.590,0.151,0.521,0.193,P values were<0.001、<0.001、<0.001、0.037、<0.001、0.006).Multiple Logistic regression analysis showed that with the control group as a reference,the risk of abnormalities in one,two,and multiple indicators of glucose and lipid metabolism indicators increased as GDF15 mRNA level increased[odds ratios(OR)were 2.03,5.47 and 18.23,P val
