摘  要:目的评估3D打印辅助自体髂骨与近端锁定钢板结合治疗的优势。方法依据适宜的筛选准则,选取2023年1月至2024年1月期间在本院进行手术治疗的30例60岁以上老年人肱骨近端骨折的患者作为研究对象。其中采用3D打印辅助自体髂骨支撑联合肱骨锁定钢板治疗15例(3D打印组),采用自体髂骨支撑技术联合肱骨锁定钢板治疗15例(传统手术组)。对比两组患者的疗效差异。结果在手术时长、出血量及手术过程中的透视次数,3D打印组相较于传统手术组表现出显著的优势,二者之间的差异具有统计学上的显著性(P<0.05)。表明3D打印技术能够有效降低手术创伤。在术后并发症发生率、骨折解剖复位率以及术后六个月肩关节CMS评分方面,两组间的差异无统计学意义(P>0.05),表明3D打印技术辅助在长期治疗效果和功能恢复方面未产生影响。结论3D打印辅助自体髂骨支撑技术与肱骨近端锁定钢板相结合的治疗方案,在降低手术时间、减少术中出血以及减少术中透视次数等方面具有明显优势。Objective To evaluate the advantage of 3D printing-assisted autologous iliac bone support combined with proximal humeral locking plate fixation.Methods Based on suitable selection criteria,30 patients over 60 years old with proximal humeral fractures who underwent surgery in our hospital from January 2023 to January 2024 were selected.Among them,15 patients were treated with 3D printing-assisted autologous iliac bone support combined with humeral locking plate fixation(3D printing group),while the other 15 were treated with traditional autologous iliac bone support combined with humeral locking plate fixation(traditional surgery group).The outcomes of the two groups were compared.Results The 3D printing group had significant advantages compared with the control group in terms of surgery duration,blood loss,and the number of intraoperative fluoroscopies,with statistically significant differences(P<0.05).This indicates that 3D printing technology can effectively reduce surgical trauma.However,there were no significant differences between the two groups in terms of postoperative complication rates,fracture anatomical reduction,or the CMS score of the shoulder joint at six months post-surgery(P>0.05),suggesting that 3D printing-assisted technology did not impact long-term treatment outcomes or functional recovery.Conclusion The treatment regimen combining 3D printing-assisted autologous iliac bone support with proximal humeral locking plate fixation offers clear advantages in reducing surgery time,intraoperative blood loss,and the number of intraoperative fluoroscopies.
