摘　　要：目的研究药品集中带量采购政策实施前后二、三级公立医院药品使用结构变化并提出优化方案。方法统计本市2家三甲医院、3家二甲医院2018—2023年药品采购情况,通过折线图、柱状图等比较同一医疗机构不同年度药品使用结构情况、不同级别医疗机构相同年度药品使用结构情况以及各级医疗机构不同年度抗菌药物、抗肿瘤药物使用前十占比情况。结果各级医疗机构集中带量采购品种数和使用金额呈上升趋势;同一级别医疗机构不同年度抗菌药物使用金额占比均呈下降趋势;不同级别医疗机构抗肿瘤药物使用金额占比有较大差异。结论集中带量采购前后各级医疗机构药品使用结构有一定程度的变化,集中带量采购政策执行较好,同时也存在一些非中选同类可替代药品使用量异常增加。Objective To study the changes in drug usage structure in second and third tier public hospitals before and after the implementation of the policy of centralized procurement with quantity,and propose optimization plans.Methods The drug procurement situation of 2 tertiary hospitals and 3 second-class hospitals in this city from 2018 to 2023 was statistically analyzed.The drug usage structure of the same medical institution in different years,the drug usage structure of different levels of medical institutions in the same year,and the top ten proportions of antibacterial and anti-tumor drugs used by different levels of medical institutions in different years were compared through line charts,bar charts,and other methods.Results The number of products of centralized procurement with quantity and the amount used by medical institutions at all levels have shown an upward trend.The proportion of antibacterial drug usage in medical institutions of the same level shows a downward trend in different years.There were significant differences in the proportion of anti-tumor drug usage in medical institutions of different levels.Conclusion There has been a certain degree of change in the drug usage structure of medical institutions at all levels before and after centralized procurement,and the policy of quantity based procurement has been well implemented.At the same time,there is also an abnormal increase in the usage of some non selected substitute drugs of the same type.
