汉防己甲素联合体外膈肌起搏治疗肺尘埃沉着病的临床效果

摘　　要：目的观察汉防己甲素与体外膈肌起搏(EDP)联合治疗肺尘埃沉着病患者的临床效果。方法选取2022年10月至2023年10月韶关市第二人民医院收治的80例男性肺尘埃沉着病患者作为研究对象,按照随机数字表法将其分为对照组(40例)与观察组(40例),对照组患者采用汉防己甲素治疗,观察组患者采用汉防己甲素联合EDP治疗。比较两组患者的症状体征积分、肺功能、血气指标,以及6 min步行试验(6MWT)。结果两组患者治疗前的肺部啰音积分、咳痰积分、第1秒用力呼气量(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC、动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2)),以及6MWT比较,差异无统计学意义(P>0.05);两组患者治疗后的肺部啰音积分、咳痰积分、PaCO_(2)均低于本组治疗前,FEV_(1)、FVC、FEV_(1)/FVC、PaO_(2),以及6MWT均高于本组治疗前,差异有统计学意义(P<0.05);观察组治疗后的肺部啰音积分、咳痰积分、PaCO_(2)均低于对照组,FEV_(1)、FVC、FEV_(1)/FVC、PaO_(2),以及6MWT均高于对照组,差异有统计学意义(P<0.05)。结论肺尘埃沉着病患者采用汉防己甲素与EDP联合治疗能够减轻患者临床症状,改善患者血气指标及肺功能,提高患者的运动耐量。Objective To observe the clinical effect of Tetrandrine combined with extracorporeal diaphragmatic pacing(EDP)in the treatment of patients with pneumoconiosis.Methods A total of 80 male patients with pneumoconiosis admitted to the Second People's Hospital of Shaoguan City from October 2022 to October 2023 were selected as the research objects.According to the random number table method,they were divided into control group(40 cases)and observation group(40 cases).The control group was treated with Tetrandrine,and the observation group was treated with Tetrandrine combined with EDP.The scores of symptoms and signs,pulmonary function,blood gas indexes and 6-min walk test(6MWT)were compared between the two groups.Results Before treatment,there were no statistically significant differences in lung rales score,expectoration score,forced expiratory volume in one second(FEV_(1)),forced vital capacity(FVC),FEV_(1)/FVC,arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2)),and 6MWT between the two groups(P>0.05).After treatment,the lung rale score,expectoration score and PaCO_(2) of the two groups were lower than those before treatment,FEV_(1),FVC,FEV_(1)/FVC,PaO_(2) and 6MWT were higher than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the lung rales score,expectoration score and PaCO_(2) of the observation group were lower than those of the control group,FEV_(1),FVC,FEV_(1)/FVC,PaO_(2) and 6MWT were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Tetrandrine combined with EDP in the treatment of patients with pneumoconiosis can reduce the clinical symptoms of patients,improve the blood gas indexes and lung function,and improve the exercise tolerance of patients.
