摘　　要：目的探讨2型糖尿病(T2DM)患者中发生糖尿病足(T2DF)的影响因素。方法选取2020年6月至2023年6月济南市长清区人民医院内分泌科收治的56例T2DM患者作为研究对象,根据有无T2DF分为A组(单纯T2DM患者,30例),B组(T2DF患者,26例)。采用t检验分析两组血清脂蛋白[LP(a)]、超敏C反应蛋白(hs-CRP)、甘油三酯、总胆固醇、低密度脂蛋白胆固醇等指标;运用logistic回归分析T2DM患者发生T2DF的影响因素。结果B组的LP(a)、hs-CRP水平高于A组,差异有统计学意义(P<0.05);LP(a)(β=1.487,OR=4.423,95%CI:1.503~12.016)和hs-CRP(β=1.912,OR=12.698,95%CI:1.068~43.274)均为T2DM患者发生T2DF的独立影响因素(P<0.05)。结论LP(a)和hs-CRP可能参与了T2DF的发病过程,二者有望成为T2DF的早期预防和监测指标。Objective To explore the influencing factors of the occurrence of type 2 diabetic foot(T2DF)in type 2 diabetes mellitus(T2DM)patients.Methods A total of 56 T2DM patients from June 2020 to June 2023 were selected as the study objects,and they were divided into Group A(patients only with T2DM,30 cases)and Group B(patients with T2DF,26 cases)according to the presence or absence of T2DF.Serum lipoprotein(a)(LP[a]),hypersensitive C-reactive protein(hs-CRP),triglycerides,total cholesterol,and low density lipoprotein cholesterol were measured in each group were analyzed by t test.The influencing factors of the occurrence of T2DF in patients with T2DM were analyzed by logistic regression.Results The levels of LP(a)and hs-CRP in Group B were higher than those of Group A,and the differences were statistically significant(P<0.05).The LP(a)(β=1.487,OR=4.423,95%CI:1.503-12.016)and hs-CRP(β=1.912,OR=12.698,95%CI:1.068-43.274)were independent influencing factors of the occurrence of T2DF in patients with T2DM(P<0.05).Conclusion LP(a)and hs-CRP may be involved in the pathogenesis of T2DF,and both are expected to be the early prevention and monitoring indicators of T2DF.
