摘　　要：目的探讨个案管理为主导的护联体服务在急性心肌梗死患者全程管理中的应用效果。方法选取2023年1月至10月无锡市人民医院收治的84例急性心肌梗死患者为研究对象,按照随机数字表法分为观察组(42例)和对照组(42例)。观察组采用以个案管理为主导的护联体服务体系进行全程追踪管理,对照组患者采用心内科一般护理常规进行护理及回访。比较两组患者再入院率、焦虑及抑郁情况、急性心肌梗死疾病相关知识掌握情况、社区满意度和信任度。结果两组患者再入院率比较,差异无统计学意义(P>0.05);观察组焦虑和抑郁评分低于对照组,差异有统计学意义(P<0.05);观察组急性心肌梗死患者知识测评量表评分、社区医院满意度评分、信任度评分均高于对照组,差异有统计学意义(P<0.05)。结论以个案管理为主导的护联体服务能够缓解急性心肌梗死患者焦虑抑郁情绪,提高患者疾病知识掌握度,提高患者对社区医院满意度、信任度。Objective To explore the application effect of case management-led nursing consortium services in the whole process management of acute myocardial infarction patients.Methods A total of 84 patients with acute myocardial infarction admitted to Wuxi People's Hospital from January to October 2023 were selected as the study objects,and they were divided into observation group(42 cases)and control group(42 cases)according to random number table method.The observation group was followed up by the case management-led nursing consortium service system,while the control group was nursed and returned by the general nursing routine of the department of cardiology.The readmission rate,anxiety and depression,knowledge of acute myocardial infarction,community satisfaction and trust were compared between the two groups.Results There was no significant difference in readmission rate between the two groups(P>0.05).The anxiety and depression scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The scores of knowledge assessment scale,community hospital satisfaction scores and trust scores of patients with acute myocardial infarction in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).Conclusion Case management-led care union service can alleviate the anxiety and depression of patients with acute myocardial infarction,improve their mastery of disease knowledge,and improve their satisfaction and trust in community hospitals.
