基于数据挖掘探析田雪飞教授运用经方及虫类中药治疗膀胱癌临证思路

摘　　要：目的基于数据挖掘分析田雪飞教授治疗膀胱癌的处方,明确其组方用药特色,以期为膀胱癌的临床辨证论治提供参考。方法收集和整理2022年1月至2024年8月湖南中医药大学第一附属医院国医堂田雪飞教授门诊诊治的膀胱癌患者的临床资料,筛选符合要求的处方信息,采用Excel建立处方数据库,采用古今医案云平台V2.3.8进行方剂频次分析、中药频次分析、功效类别分析、性味及归经分析、关联规则分析,采用SPSS Statistics 26.0进行高频药物聚类分析,采用R语言进行复杂网络分析。结果共纳入222张处方,涉及方剂62首,总频次778次,使用频率>8%的高频方剂有当归贝母苦参丸、龙虎散、滑石白鱼散等13首。涉及中药178味,用药频次共计3459次,使用频率>25%的高频药物包括当归、血余炭、壁虎、苦参、茯苓等22味,使用频率>5%的虫类中药包括壁虎、乌梢蛇、蜂房等8味。药物功效类别主要为补虚药、清热药,药性以寒、平为主,药味多属甘、苦、辛,主归肝、脾、胃经。关联规则分析得到12组常用药对。聚类分析得到4类药物组合。复杂网络分析得到13味核心药物。结论田教授诊治膀胱癌以六经辨证为纲,形成“擅用经方,精方围治”的组方特色,以及“巧用虫类,妙用炮制,精于用量”的用药特点。Objective To analyze prescription of Professor Tian Xuefei’s in the treatment of bladder cancer based on data mining,clarify the characteristics of its prescription and medication,so as to provide reference for clinical treatment based on syndrome differentiation of bladder cancer.Methods Clinical data of patients with bladder cancer in Outpatient Clinic of Professor Tian Xuefei,Guoyi Hall,the First Affiliated Hospital of Hunan University of Chinese Medicine from January 2022 to August 2024 were collected and sorted.Prescription information that meets the requirements was screened.The prescription database was established by using Excel.The prescription frequency analysis,traditional Chinese medicine frequency analysis,efficacy category analysis,nature,flavour and channel tropism analysis,and association rules analysis were implemented by using the ancient and modern medical case cloud platform V2.3.8.Cluster analysis of high-frequency drugs was conducted by using SPSS Statistics 26.0.Complex network analysis was conducted by using R language.Results A total of 222 prescriptions were included,involved 62 prescriptions,and total frequency of prescription was 778 times.High-frequency prescriptions used more than 8%included 13 prescriptions such as Danggui Beimu Kushen Pills,Longhu Powder,Huashi Baiyu Powder,etc.A total of 178 kinds of traditional Chinese medicines were involved,with total frequency of 3459 times,and 22 kinds of high-frequency drugs with frequency more than 25%included 22 herbs such as Angelicae Sinensis Radix,Carbonized Hair,Gecko,Sophorae Flavescentis Radix,Poria,etc.The insect Chinese medicine with frequency of more than 5%included eight flavors such as Gecko,Black-Tail Snake,Honeycomb,etc.The main categories of drug efficacy were tonifying deficiency drugs,and clearing heat drugs.The medicinal properties were mainly cold and flat,and the flavors were mostly sweet,bitter,and pungent,mainly targeting liver,spleen,and stomach meridians.Association rule analysis obtained 12 groups of common drug p
