津沽疮疡学派高思敬论治疔疮思想探析

摘　　要：以《外科医镜》为依托,探讨清代津沽疮疡学派医家高思敬的学术思想。高思敬以三因统领疔疮病因,分为外因、内因、不内外因。外因责之邪气侵袭,趁虚而入;内因责之七情内伤,气郁化火;不内外因责之饮食不节、房劳过度,脏腑积热,三者共同构成疔疮的发病机制。在治疗时,高思敬以诊察顺逆,详辨阴阳,重视脏腑,兼顾经络的辩证思想贯穿疔疮诊疗始终;临证以内外合治、以清为要,辨脓深浅、莫畏针刀,毒邪入营、专药专用为其临证特色;用药方面,总以清火败毒为原则,擅以菊花饮据症灵活化裁,并独创甘露根治疔疮走黄重症之法;刀针方面,其于辨脓之深浅,成熟与否颇有心得,并对刺脓时进刀针的深度提出规范化标准。高思敬临证时外症内治,方药灵活化裁,随症用之,形成自己的治疔特色。Based on Surgical Mirror,this paper probes into academic thought of Gao Sijing,a doctor of Jingu Sore and Ulcer School in Qing Dynasty.Gao Sijing leds the cause of hard furuncle with three causes,which are divided into external cause,internal cause,and non-endo-non-exogenous cause.The evil attack of external cause,and took advantage of weakness;internal cause due to the seven feelings of internal injury,qi depression fire;non-endo-non-exogenous cause blames of improper diet,excessive room labor,zang fu viscera heat accumulation,the three together constitute the pathogenesis of hard furuncle.During treatment,Gao Sijing’dialectical thinking of diagnosing order and reverse,distinguishing yin and yang in detail,paying attention to zang fu viscera,and taking into account meridians runs through the diagnosis and treatment of hard furuncle.Clinical syndromes include internal and external combined treatment,mainly focuse on clearing,distinguish the depth of pus,fearlessness of needle and knife,poison and evil entering the camp,and special medicine;in terms of medication,based on the principle of clearing fire and eliminating poison,skilled in using Juhua Yin to flexibly adjust symptoms,and creates a unique method of using Ganlugen to cure severe hard furuncle and eliminate jaundice;in terms of needle and knife,distinguishing the depth of pus,have a lot of experience in maturity or not,and propose standardized standards for the depth of surgical needle during pus injection.Gao Sijing clinical syndrome when external disease internal treatment,the prescription is flexible and activated,with the disease,formes unique characteristics in the treatment of hard furuncle.
