王彦刚教授基于“核心病机观”辨证论治手足逆冷常用角药

摘　　要：医家多从虚、寒论治手足逆冷,然临床以补虚散寒之法收效不佳。随着人们饮酒、煎炸炙煿之品摄入增多等饮食习惯的改变,王彦刚教授认为手足逆冷患者的核心病机多为“湿热中阻,阳郁不通”。湿热郁久易化生痰浊、瘀血等有形实邪,加重病情,故王教授在以清利湿热为核心治法的基础上,兼以活血化瘀、清热化痰、补脾扶正之法。临证中重视四诊合参、整体观念,在辨证论治的基础上,善用角药,灵活加减,疗效显著。特附验案1则以佐验。Doctors often in the treatment of hand and foot reverse cold from the perspectives of deficiency and coldness,but the effect of supplementing deficiency and dispelling cold is not good in clinical practice.With changes in people’s dietary habits such as drinking alcohol,and consuming more fried and baked goods,Professor Wang Yangang believes that the core pathogenesis of patients with hand and foot reverse cold is often“dampness-heat obstruction,and yang depression obstruction”.Long term dampness-heat obstruction can easily transform into tangible solid pathogens such as phlegm turbidity,and blood stasis,exacerbating condition.Therefore,Professor Wang,based on the core treatment method of clearing dampness-heat,also uses methods of promoting blood circulation for removing blood stasis,clearing heat and dissipating phlegm,and tonifying spleen and supporting body.In clinical practice,attach importance to comprehensive analysis of data gained by four diagnostic methods,and holism.On the basis of syndrome differentiation and treatment,horn drugs are used well,with flexible addition and subtraction,and therapeutic effect is significant.Attached case one is verified.
