摘 要:目的探究加温加湿二氧化碳气腹在妇科腹腔镜手术中对患者体温调控及凝血功能的影响。方法选取98例行妇科腹腔镜手术的患者,根据随机数字表法分为研究组与对照组,各49例。研究组将二氧化碳加温至37℃,加湿至90%;照组二氧化碳处于室温,湿度为0。比较两组患者体温与凝血功能指标。结果术后两组体温均逐渐降低,但研究组术后15min、30min、60min、100min及手术结束时的体温均高于对照组,差异有统计学意义(P<0.05)。同时点组间四项凝血指标比较,差异无统计学意义(P>0.05)。术前与术后100min组内比较,凝血酶原时间高于术前,活化部分凝血活酶时间与纤维蛋白原水平均低于术前,差异有统计学意义(P<0.05)。结论腹腔镜术中建立气腹时,对二氧化碳进行加温加湿可有效改善患者体温下降,而凝血功能与二氧化碳加温加湿的相关性不明显。Objective To explore the effects of heated and humidified carbon dioxide pneumoperitoneum on body temperature regulation and coagulation function in gynecological laparoscopic surgery.Methods 98 patients undergoing laparoscopic surgery were selected and randomly divided into a study group and a control group,with 49 cases in each group.The study group heated the carbon dioxide to 37℃ and humidified it to 90%,while the control group kept carbon dioxide at room temperature and humidity at 0%.The body temperature and coagulation function indicators between the two groups of patients were compared.Results The body temperature of the two groups gradually decreased after surgery,but the study group had higher body temperature than the control group at 15 minutes postoperatively,30 minutes postoperatively,60 minutes postoperatively,100 minutes postoperatively,and at the end of surgery(P<0.05).At the same time,there were no statistically significant differences in the comparison of four coagulation indicators between groups(P>0.05).Compared within the group before and 100 minute after surgery,the prothrombin time was higher than preoperative and the activated partial thromboplastin time and fibrinogen level were lower than preoperative(P<0.05).Conclusion When establishing pneumoperitoneum during laparoscopic surgery,heating and humidification carbon dioxide can effectively reduce the decrease in patient's body temperature,while coagulation function is not significantly affected by whether carbon dioxide is heated or humidified.
