腹腔镜手术治疗异位妊娠的疗效及对再发异位妊娠的影响观察

摘　　要：目的探讨腹腔镜手术治疗异位妊娠的疗效及对再发异位妊娠的影响。方法选取80例异位妊娠患者,将行腹腔镜下输卵管开窗取胚术联合甲氨蝶呤治疗的40例纳入对照组,行腹腔镜下患侧输卵管切除术治疗的40例纳入试验组。比较两组临床症状缓解情况、手术前后血人绒毛膜促性腺激素(HCG)水平,并统计治疗后1年内再发异位妊娠情况。结果试验组总有效率高于对照组,差异有统计学意义(P<0.05)。术后第2天,两组血HCG水平明显降低,且试验组降低幅度更明显,差异均有统计学意义(P<0.05)。试验组治疗后1年内再发异位妊娠率低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜下患侧输卵管切除术治疗异位妊娠的疗效显著,可降低血HCG水平,降低再发异位妊娠发生率。Objective To explore the therapeutic effect of laparoscopic surgery on patients with ectopic pregnancy and its impact on recurrent ectopic pregnancy.Methods 80 patients with ectopic pregnancy were selected,and 40 patients who underwent laparoscopic salpingostomy combined with methotrexate treatment were included in the control group,40 patients who underwent laparoscopic salpingectomy on the affected side were included in the experimental group.The clinical symptom relief,preoperative and postoperative blood chorionic gonadotropin(HCG)levels between the two groups were compared,and the incidence of recurrent ectopic pregnancy within one year after treatment were compared.Results The total effective rate of the experimental group was higher than that of the control group(P<0.05).On the second day after surgery,the blood HCG levels in both groups significantly decreased,and the experimental group showed more significant decrease(P<0.05).The recurrence rate of ectopic pregnancy within 1 year after treatment in the experimental group was lower than that in the control group(P<0.05).Conclusion Laparoscopic salpingectomy has a significant therapeutic effect on patients with ectopic pregnancy,which can reduce blood HCG level and lower the incidence of recurrent ectopic pregnancy.
