人工点阵CO_(2)激光联合皮损内注射治疗瘢痕疙瘩1例

摘　　要：分析1例瘢痕疙瘩患者进行人工点阵CO_(2)激光联合皮损内注射治疗的效果。患者女性,39岁,右侧指腕处瘢痕挛缩畸形1年余。行5次皮损内注射(盐酸利多卡因注射液+复方倍他米松注射液+注射用氟尿嘧啶)后,瘢痕大小、隆起高度及痛痒等症状未见明显缓解,质硬,颜色鲜红,且患者自觉瘢痕处有向外扩张趋势,温哥华瘢痕量表(VSS)评分12分,患者与观察者瘢痕评估量表(POSAS)中观察者评分43分,患者评分60分。患者考虑仅单纯行药物治疗效果欠佳,故改用人工蜂窝状点阵CO_(2)激光联合皮损内注射,45天1次,治疗4次后,瘢痕明显变小,质地变软,VSS评分7分,POSAS观察者评分31分,患者评分39分。目前该患者仍在治疗中,且无明显不良反应。This paper is to analyze the effect of artificial fractional CO_(2) laser combined with intralesional injection therapy on a patient with keloid.A 39-year-old female patient has suffered from scar contracture deformity on her right wrist for over a year.After the initial 5 injections of lidocaine hydrochloride,compound betamethasone,and fluorouracil into the skin lesions,there was no significant improvement in scar size,protrusion height,pain,and itching symptoms.The scar was hard in texture,bright red in color,and the patient felt that the scar had a tendency to expand outward.The initial scores were 12 on the Vancouver Scar Scale(VSS)and 43(observer)and 60(patient)on the Patient and Observer Scar Assessment Scale(POSAS).Due to the patient's dissatisfaction with the drug therapy alone,the treatment therapy was switched to a combination of artificial fractional CO_(2) laser and intralesional injections once every 45 days.After 4 treatments,the scars significantly decreased in size and texture,with a VSS score of 7 points,a POSAS score of 31(observer)and 39(patient).At present,the patient is still under treatment and has no significant adverse reactions reported.
