半夏白术天麻汤治疗痰浊中阻型后循环缺血性眩晕的研究

摘　　要：目的:探究半夏白术天麻汤治疗痰浊中阻型后循环缺血性眩晕的价值。方法:回顾性分析2020年1月—2024年3月在河南省商丘市中医院接受治疗的60例痰浊中阻型后循环缺血性眩晕患者的临床资料,依据治疗方法分为参照组、研究组,每组各30例。两组均应用甲磺酸倍他司汀治疗,同时研究组增加使用半夏白术天麻汤治疗。对比两组的临床疗效、中医证候积分、超声检查结果、不良反应。结果:与参照组患者相比,研究组患者的治疗总有效率更高(P <0.05)。治疗后,与参照组患者相比,研究组患者的中医证候积分、基底动脉血管搏动指数(PI)、左侧椎动脉PI、右侧椎动脉PI更低(P <0.05),基底动脉平均血流速度(Vm)、左侧椎动脉Vm、右侧椎动脉Vm更高(P <0.05)。两组患者不良反应的总发生率对比,差异无统计学意义(P> 0.05)。结论:半夏白术天麻汤可有效改善痰浊中阻型后循环缺血性眩晕患者的脑循环,减轻其临床症状,获得显著的治疗效果,值得临床推广。Objective:To explore the value of Banxia Baizhu Tianma Decoction in the treatment of posterior circulation isc hemic vertigo with phlegm turbidity obstructing type.Methods:Clinical data of 60 patients with post-circulation ischemic vertigo treated in Shangqiu Hospital of Traditional Chinese Medicine from January 2020 to March 2024 were retrospectively analyzed.According to treatment methods,they were divided into a reference group and a study group,with 30 cases in each group.Both groups were treated with Betahistine mesylate,while the study group was treated with Betahistine mesylate plua Banxia Baizhu Tianma Decoction.The clinical efficacy,TCM syndrome score,ultrasonic examination results and adverse reactions of the two groups were compared.Results:After treatment,compared with the reference group,the total effective rate of the study group was higher(P<0.05),and the TCM syndrome score,basilar artery pulastility index(PI),left vertebral artery PI and right vertebral artery PI of the study group were lower(P<0.05).Basal artery mean flow velocity(Vm),left vertebral artery Vm,right vertebral artery Vm of the study group were higher(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Banxia Baizhu Tianma Decoction can effectively improve the cerebral circulation of patients with post-circulation ischemic vertigo,alleviate their clinical symptoms,and achieve significant therapeutic effects,which is worthy of clinical promotion.
