透热转气法在重症急性胰腺炎的应用探讨

摘　　要：重症急性胰腺炎是消化内科危急重症,病机复杂,涉及多个脏腑。基于卫气营血辨证,以透热转气为理论指导,结合重症急性胰腺炎病因病机及病理因素,把握疾病病理阶段与中医“营分”理论病机病位、病性病势高度契合,在清营热、养营阴基础上,开达、宣透、清热、祛湿、化瘀、解毒,以调畅气机,透热转气,透邪外出,以探讨重症急性胰腺炎辨证论治新思路。Severe acute pancreatitis is a critical and severe disease in digestive medicine,with complex pathogenesis and involving multiple viscera.Based on the syndrome differentiation according to the four processes of Weifen,Qifen,Yingfen and Xuefen,guided by the theory of clearing heat of Yingfen to Qifen,taking into account the etiology,pathogenesis and pathological factors of severe acute pancreatitis,the paper holds that the pathological stage of the disease is highly consistent with the pathogenesis,location and nature of the disease according to the theory of"Yingfen"in traditional Chinese medicine.On the basis of clearing Yingfen heat and nourishing Yingfen yin,therapies are applied to activate membrane,dispel pathogens,clear heat,dispel dampness,remove blood stasis and resolve toxins,so as to promote qi movement,clear heat of Yingfen to Qifen,and eliminate pathogens.This paper is aimed at exploring the new approach for syndrome differentiation and treatment of severe acute pancreatitis.
