摘　　要：目的观察基于“皮-肺-肠”轴理论中药保留灌肠治疗变态反应性皮肤病的临床疗效。方法选择湿疹、特应性皮炎、慢性荨麻疹患者各80例,随机将每种疾病患者各分为观察组与对照组,每组各40例。治疗过程中,荨麻疹观察组1例灌肠后腹痛不适,停药后缓解,退出研究。各对照组予维生素C注射液、葡萄糖酸钙注射液静脉滴注,盐酸左西替利嗪片口服等常规治疗方法治疗。各观察组在对照组基础上联合中药保留灌肠治疗。各组疗程均15 d。比较各组皮损及瘙痒缓解情况,IL-4、总IgE、EOS等变态反应性指标水平。记录各组不良反应发生情况。结果治疗后,各病种观察组瘙痒改善程度明显优于对照组(P<0.05);湿疹、特应性皮炎观察组EASI评分改善程度优于对照组(P<0.05);慢性荨麻疹USA7评分改善程度优于对照组(P<0.05);各观察组血清变态反应性指标改善程度均优于对照组(P<0.05)。结论葛根芩连汤加味灌肠方保留灌肠治疗湿疹、特应性皮炎、慢性荨麻疹等变态反应性疾病安全有效。Objective To observe the clinical effect of retention enema in the treatment of patients with allergic skin diseases based on the"skin-lung-intestine"axis theory.Methods A total of 80 cases of eczema,80 cases of atopic dermatitis and 80 cases of chronic urticaria were randomly divided into two groups,with 40 cases in the observation groups and 40 cases in the control groups,respectively.The control groups were treated with conventional treatment methods such as vitamin C injection,calcium gluconate injection and(R)-cetirizine tablets,while the observation groups were treated with retention enema on the basis of routine treatment,once a day.All the groups were treated for 15 days.The relief of skin lesions and itching in each group,levels of IL-4,total IgE and EOS and other allergenic indicators were compared in each group,and their adverse reactions were recorded.Results During the course of treatment,a patient in the urticaria group had abdominal pain after enema,which was relieved after drug withdrawal and the patient withdrew from the study.After treatment,the improvement degree of pruritus in the observation groups was better than that in the control groups(P<0.05),and the improvement degree of EASI scores in the observation groups of eczema and atopic dermatitis was better than that in the control groups(P<0.05),the improvement degree of USA7 scores in the chronic urticaria group was better than that in the control group(P<0.05),the improvement degree of serum allergic indexes in all the observation groups was better than that in the control groups(P<0.05).Conclusion Retention enema with modified Gegen Qinlian decoction is safe and effective in the treatment of allergic dermatosis such as eczema,atopic dermatitis and chronic urticaria.
