电针水沟穴促进缺血性脑卒中后血管新生机制研究

摘　　要：目的探讨电针水沟穴对大鼠缺血性脑卒中后缺血半暗带血管内皮细胞增殖情况和内皮型一氧化氮合酶(eNOS)表达的影响。方法选取45只健康SPF级雄性SD大鼠制备大脑中动脉闭塞(MCAO)模型,随机分为假手术组、模型组及电针组。采用Longa评分评价神经功能缺损体征;HE染色观察大鼠脑皮质病理形态;免疫荧光检测大鼠脑缺血半暗带微血管密度和血管内皮细胞增殖情况;实时荧光定量PCR检测eNOS mRNA相对表达量。结果与假手术组相比较,模型组大鼠神经功能缺损体征评分与脑梗死体积比均上升(P<0.01),缺血半暗带微血管密度增大(P<0.01),CD31^(+)/Ki67^(+)阳性细胞数上升(P<0.01),eNOS mRNA表达降低(P<0.05)。与模型组大鼠比较,电针组大鼠神经功能缺损体征评分降低(P<0.01),脑梗死体积比减少(P<0.05),脑微血管密度进一步增大(P<0.05)。CD31^(+)/Ki67^(+)阳性细胞数进一步上升(P<0.05),eNOS mRNA表达水平上升(P<0.05)。结论电针水沟治疗可改善神经功能缺损症状,上调eNOS表达,增强缺血半暗带血管内皮细胞增殖,促进缺血性脑卒中大鼠血管新生。Objective To explore the effects of electroacupuncture(EA)at the Shuigou acupoint on the proliferation of vascular endothelial cells in the ischemic penumbra and the expression of endothelial nitric oxide synthase(eNOS)in a rat model of ischemic stroke.Methods Forty-five healthy male SD rats(SPF)were subjected to the middle cerebral artery occlusion(MCAO)procedure to induce the ischemic stroke model.Then,the rats were randomly divided into a sham group,a MCAO group,and an EA group.Longa score was used to evaluate the neurological deficits.The pathological morphology of the rat cerebral cortex was observed by HE staining.The density of microvessels and proliferation of vascular endothelial cells in ischemic penumbra were detected by immunofluorescence.RT-qPCR was used to detect the relative mRNA expression of eNOS.Results Compared to the sham group,rats in the MCAO group showed increased neurological deficit scores and cerebral infarction volumes(P<0.01),the microvascular density in the ischemic penumbra increased(P<0.01),CD31^(+)/Ki67^(+)positive cells increased(P<0.01),and the mRNA expression of eNOS decreased(P<0.05).Compared to the MCAO group,the neurological deficits scores of rats in the EA group decreased(P<0.01),the volume ratio of cerebral infarction decreased(P<0.05),and the microvascular density further increased(P<0.05).The number of CD31^(+)/Ki67^(+)positive cells increased further(P<0.05),and the mRNA expression level of eNOS increased(P<0.05).Conclusion Electroacupuncture at the Shuigou acupoint can ameliorate neurological functional deficits,up-regulate the expression of eNOS,consequently enhancing the proliferation of vascular endothelial cells in the ischemic penumbra,thereby promoting angiogenesis in rats with ischemic stroke.
