TurboHawk斑块旋切系统治疗膝下动脉硬化闭塞症的短中期临床疗效研究

摘　　要：目的评价TurboHawk斑块旋切系统治疗膝下动脉硬化闭塞症的短中期疗效。方法回顾性分析2018年1月至2022年2月南京大学医学院附属鼓楼医院使用TurboHawk斑块旋切术治疗膝下动脉硬化闭塞症的17例患者(18条患肢)临床资料,观察手术成功率及并发症发生情况,比较手术前后间歇性跛行距离、踝肱指数、临床驱动的靶病变血运重建率。结果手术技术成功率为94.44%(17/18),无手术相关并发症及不良事件出现。患者术后随访7.03~62.30个月,平均(50.19±5.89)个月。术后1周踝肱指数和6个月间歇性跛行距离均优于术前,差异有统计学意义(P<0.001)。患者免于截肢率为100%(18/18),全因死亡率为5.88%(1/17),3例患者出现靶血管再狭窄并再次接受血运重建术。术后36、48个月免于临床驱动的靶病变重建率分别为88.89%(16/18)、83.33%(15/18),临床持续改善率分别为88.89%(16/18)、77.78%(14/18)。结论TurboHawk斑块旋切系统治疗膝下动脉硬化闭塞症安全且具有良好的短中期疗效。Objective To evaluate the short-and mid-term efficacy of TurboHawk plaque rotation system in the treatment of arteriosclerotic obliterans in the infrapopliteal arteries.Methods The clinical data of 17 patients(18 limbs)with arteriosclerotic obliterans in the infrapopliteal arteries who underwent TurboHawk atherectomy in the affiliated Drum Tower Hospital,Medical School of Nanjing University from January 2018 to February 2022 were analyzed retrospectively.The surgical success rate and complications were observed,and intermittent claudication distance,ankle-brachial index and clinically driven target limb reintervention rate before and after treatment were compared.Results The technical success rate was 94.44%(17/18),and no treatment-related complications or adverse events occurred.The patients were followed up for 7.03-62.30 months after treatment,with an average of 50.19±5.89 months.The ankle-brachial index at one week postoperatively and intermittent claudication distance at 6 months postoperatively were both better than those before treatment,with statistically significant differences(P<0.001).The amputation-free rate was 100%(18/18),the all-cause mortality was 5.88%(1/17).Three patients had target vessel restenosis in the followed-up period and underwent revascularization.The rates of freedom from clinically driven target limb reintervention at 36 and 48 months were 88.89%(16/18)and 83.33%(15/18),respectively,and the rates of sustained clinical improvement were 88.89%(16/18)and 77.78%(14/18),respectively.Conclusion TurboHawk plaque rotation system is safe and has good short-and mid-term efficacy in the treatment of arteriosclerotic obliterans in the infrapopliteal arteries.
