从大气下陷论治中风合并阻塞性睡眠呼吸暂停低通气综合征

摘　　要：缺血性脑卒中与阻塞性睡眠呼吸暂停低通气综合征(OSAHS)存在共同危险因素,常共同发病。OSAHS作为缺血性脑卒中独立的危险因素,亦可加重再次中风的风险,影响预后及康复。基于近代名医张锡纯“大气下陷”理论体系,认为大气虚陷,气血失调是中风合并OSAHS的病机根本,并从大气虚滞,清窍不利、内毒积蕴,大气亡脱阐释病进机制。临证以益气升提为主要制方思路,病于痰瘀者,治以温补心肺,开通玄府;病于内毒者,治以启闭解毒,补敛固脱,以期为改善中风合并OSAHS的治疗提供新思路。There are common risk factors for ischemic stroke and obstructive sleep apnea hypopnea syndrome(OSAHS).OSAHS,as an independent risk factor for ischemic stroke,can also increase the risk of repeat stroke and affect prognosis and rehabilitation.Based on the pectoral qi subsidence theory provided by Zhang Xichun,a famous physician in recent times,it is believed that the pectoral qi subsidence and disorder of qi and blood are the root causes of stroke combined with OSAHS,explaining the disease mechanism from the qi deficiency and stagnation,blocked clear orifice,toxic heat stagnating in the body,and qi exhaustion.To improve the condition of stroke combined with OSAHS,the main idea of benefiting qi and elevating qi should be applied in clinical practice.The patients with phlegm and stagnation should be treated by warming and tonifying the heart and lungs to open sweat pore,while the patients with internal toxins should be treated by breaking the block and opening the orifices to resolve the toxins,and restoring deficiency to relieve depletion,to provide new ideas for improving the condition of stroke combined with OSAHS.
