摘 要:目的 探讨低强度脉冲超声(LIPUS)干预对急性心肌梗死诱导的心肌损伤的作用及机制。方法 选取雄性、健康的8周龄SPF级C57BL/6小鼠88只,按照随机数字表法将其分为假手术组、急性心肌梗死组、LIPUS干预组、LIPUS+氯喹(CQ)干预组,每组22只。假手术组只进行开胸、关胸及假治疗处理(探头无能量输出),其余三组结扎心脏冠状动脉左前降支制备小鼠急性心肌梗死模型,结扎后左前降支供血区域迅速发白且跳动减弱,与正常心肌有明显的分界线形成,说明模型制备成功。LIPUS干预组、LIPUS+CQ干预组于术前24、12 h,术后12、24 h进行LIPUS治疗,每次30 min。LIPUS+CQ干预组于术前2 h腹腔注射CQ,剂量为10 mg/kg。观察小鼠生存情况;比较各组术后28 d左室射血分数(LVEF);比较各组术后3 d血清白细胞介素(IL)-6、IL-1β、肿瘤坏死因子-α(TNF-α)水平、超氧化物歧化酶(SOD)活性及丙二醛(MDA)含量;比较各组LC3-Ⅱ、p62蛋白表达。结果 与假手术组比较,急性心肌梗死组生存率、LVEF降低(P<0.05);与急性心肌梗死组比较,LIPUS干预组生存率、LVEF升高(P<0.05);与LIPUS干预组比较,LIPUS+CQ干预组生存率、LVEF降低(P<0.05)。与假手术组比较,急性心肌梗死组IL-6、IL-1β及TNF-α水平升高(P<0.05);与急性心肌梗死组比较,LIPUS干预组IL-6、IL-1β及TNF-α水平降低(P<0.05);与LIPUS干预组比较,LIPUS+CQ干预组IL-6、IL-1β及TNF-α水平升高(P<0.05)。与假手术组比较,急性心肌梗死组MDA含量、LC3-Ⅱ蛋白表达升高,SOD活性、p62蛋白表达降低(P<0.05);与急性心肌梗死组比较,LIPUS干预组MDA含量、p62蛋白表达降低,SOD活性、LC3-Ⅱ蛋白表达升高(P<0.05);与LIPUS干预组比较,LIPUS+CQ干预组MDA含量、LC3-Ⅱ蛋白表达、p62蛋白表达升高,SOD活性降低(P<0.05)。结论 LIPUS可以改善急性心肌梗死诱导的心肌损伤,其作用机制与调节自噬有关。Objective To investigate the effect and mechanism of low-intensity pulsed ultrasound(LIPUS)on myocardial injury induced by acute myocardial infarction.Methods A total of 88 male,healthy 8-week-old SPF C57BL/6 mice were selected,and they were divided into sham operation group,acute myocardial infarction group,LIPUS intervention group,and LIPUS+Chloroquine(CQ)intervention group according to random number table method,with 22 mice in each group.In the sham operation group,only performed chest opening,chest closing,and sham treatment were performed(the probe had no energy output).The remaining three groups ligation the left anterior descending branch of the heart coronary artery to prepare a mouse acute myocardial infarction model.After ligation,the blood supply area of the left anterior descending branch quickly turned white and beat weakened,and there was a clear dividing line with normal myocardium,indicating that the model was successfully prepared.LIPUS intervention group and LIPUS+CQ intervention group were treated with LIPUS at 24,12 h before surgery and 12,24 h after surgery for 30 min each time.The LIPUS+CQ intervention group was intraper-itoneally injected CQ at 2 h before surgery,with 10 mg/kg.The survival of mice was observed;left ventricular ejection fraction(LVEF)at 28 days after surgery was compared among all groups;the levels of interleukin(IL)-6,IL-1β,tumor necrosis factor-α(TNF-α),the activity of superoxide dismutase(SOD),and the content of malondialdehyde(MDA)in serum of all groups were compared at 3 days after surgery;and the expression of LC3-Ⅱand p62 protein in each group was compared.Results Compared with sham operation group,the survival rate and LVEF in acute myocardial infarction group were decreased(P<0.05);compared with acute myocardial infarction group,the survival rate and LVEF in LIPUS intervention group were increased(P<0.05);and compared with LIPUS intervention group,the survival rate and LVEF in LIPUS+CQ intervention group were decreased(P<0.05).Compared with sham operation grou
