循经针刺郄穴治疗原发性三叉神经痛的效果及对血清P物质、降钙素基因相关肽水平的影响

摘　　要：目的 探讨循经针刺郄穴治疗原发性三叉神经痛(PTN)的效果及对血清P物质、降钙素基因相关肽(CGRP)水平的影响。方法 选取2023年6月至2024年6月黑龙江中医药大学附属第二医院针灸二科收治的60例PTN患者,按照随机数字表法将其分为对照组和郄穴组,每组30例。对照组采用常规针刺,郄穴组在对照组基础上,根据扳机点位置加刺相应郄穴,5 d为1个疗程,共治疗2个疗程。比较两组视觉模拟评分法(VAS)评分、汉密尔顿焦虑量表(HAMA)评分及血清P物质和CGRP水平。结果 郄穴组临床疗效优于对照组(P<0.05)。治疗前,两组VAS评分、HAMA评分及血清P物质和CGRP水平比较,差异无统计学意义(P>0.05);治疗后,两组VAS评分、HAMA评分及血清P物质和CGRP水平均低于治疗前,且郄穴组低于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 循经针刺郄穴治疗PTN扳机点疼痛可有效减轻患者疼痛,缓解其焦虑情绪程度,降低其血清P物质和CGRP水平。Objective To explore the effect of acupuncture along meridian with xi-cleft point in the treatment of primary trigeminal neuralgia(PTN)and effect on serum substance P and calcitonin gene-related peptide(CGRP)levels.Methods A total of 60 patients with PTN treated in the Second Department of Acupuncture of the Second Affiliated Hospital of Heilongjiang University of Chinese Medicine from June 2023 to June 2024 were selected,and they were divided into the control group and the xi-cleft point group according to random number table,with 30 cases in each group.The control group received conventional acupuncture,while the xi-cleft point group received add puncture corresponding xi-cleft point according to trigger point position based on the control group.Five days were one course of treatment,for a total of two courses of treatment.The scores of visual analogue scale(VAS),Hamilton anxiety scale(HAMA),the levels of serum substance P and CGRP between the two group were compared.Results Clinical efficacy of the xi-cleft point group was better than that of the control group(P<0.05).Before treatment,there were no statistically significant differences in scores of VAS,HAMA,and serum levels of substance P and CGRP between the two groups(P>0.05).After treatment,scores of VAS,HAMA,and serum levels of substance P and CGRP in the two groups were lower than before treatment,and the xi-cleft point group was lower than the control group(P<0.05).There were no statistically significant differences in the incidence rate of adverse reactions between the two groups(P>0.05).Conclusion Acupuncture along meridian with xi-cleft point in the treatment of trigger point pain of PTN can effectively alleviate patient pain,alleviate their anxiety level,and reduce their serum levels of substance P and CGRP.
