摘　　要：甲状腺功能减退症是临床常见的甲状腺疾病之一,目前主要以激素替代或补充治疗为主,但存在需要终身服药和具有不良反应等问题。柳红芳教授根据数十年临床经验,将“形气神”理论用于甲状腺功能减退症治疗,建立“肝肾-脾胃-心神”三焦关系轴,认为下焦肝肾精血形质亏损是发病基础,中焦脾胃气机郁滞是发展重要因素,最终造成上焦心神失养等一系列虚损性表现。治疗上,柳教授紧扣“复形-化气-调神”3个层次,遣方用药方面根据张景岳学术思想,多靶点、全过程辨证论治甲状腺功能减退症,分别应用精血双补以充形质,畅达枢机以复气机,扶阳散霭以安神志,使形充、气畅、神安,从整体上缓解患者的临床症状,疗效确切。附1则验案加以佐证。Hypothyroidism is one of the common clinical thyroid diseases,currently mainly treated with hormone replacement or supplementation,but there are problems such as the need for lifelong medication and adverse reactions.Based on decades of clinical experience,theory of“form,qi,and spirit”applied in the treatment of hypothyroidism,establish sanjiao relationship axis of“liver,kidney-spleen,stomach-heart,spirit”.Professor Liu Hongfang believes that the deficiency of liver,kidney,essence,blood,and substance in lower jiao is the basis of the disease,while the stagnation of spleen,stomach,and qi in middle jiao is an important factor in development of the disease,ultimately leading to a series of deficiency manifestations such as loss of heart and spirit in upper jiao.In terms of treatment,Professor Liu closely follows three levels of“restore form,transform qi,and regulate spirit”,in terms of prescription and medication,based on Zhang Jingyue’s academic ideas,hypothyroidism treatment based on syndrome differentiation through multi-target and whole process,applying essence and blood to replenish form,unimpeded access pivot to restore qi,and supporting yang and dispersing mist to tranquilize spirit,relieve the patient’s clinical symptoms as a whole by replenishing form,restoring qi,and calming spirit,with definite efficacy.A case study is attached to support these.
