基于案例库的血液病科住院医师规范化培训模式的应用及效果评价

摘　　要：目的探讨基于案例库的血液科住院医师规范化培训(简称住培)模式的应用效果,评估其对提升学员诊断能力和临床决策水平的影响。方法选取2023年7月—2024年6月山东第二医科大学附属医院的64名血液科住培学员,采用不同的培训模式分为观察组(n=32)和对照组(n=32)。对照组接受常规住培,观察组采用基于案例库的培训模式,包括典型病例分析、虚拟患者模拟和临床决策训练。评价两组学员的疾病诊断准确率、治疗方案制定合理性、临床思维能力评估和对培训方法的满意度。结果观察组疾病诊断准确率(92.4±3.6)分,治疗方案制定合理性(89.6±3.9)分和临床思维能力评分(88.5±3.7)分,高于对照组的(85.7±4.2)分、(82.3±4.5)分和(83.2±4.2)分,差异有统计学意义(t=6.853、6.924、5.412,P均<0.05)。观察组在培训满意度各维度评分均高于对照组,差异有统计学意义(P均<0.05)。结论基于案例库的血液科住培模式能有效提高学员的临床诊疗能力。Objective To investigate the effectiveness of a case library-based standardized residency training(SRT)model in hematology and evaluate its impact on improving trainees´diagnostic abilities and clinical decision-making skills.Methods Sixty-four hematology SRT trainees from July 2023 to June 2024 at the Affiliated Hospital of Shandong Second Medical University were divided into an intervention group(n=32)and a control group(n=32)by different training modes.The control group received conventional SRT,while the intervention group adopted a case library-based training model,including typical case analysis,virtual patient simulation,and clinical decision-making training.The accuracy of disease diagnosis,the rationality of treatment plan formulation,the evaluation of clinical thinking ability and the satisfaction of training methods were evaluated in the two groups.Results The scores of the accuracy of disease diagnosis in the observation group was 92.4±3.6,rationality of treatment plan formulation was 89.6±3.9,and scores of clinical thinking ability was 88.5±3.7,which were higher than 85.7±4.2,82.3±4.5 and 83.2±4.2 in the control group,the differences were statistically significant(t=6.853,6.924,5.412,all P<0.05).The scores of each dimension of training satisfaction in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion The resident training mode of hematology department based on case base can effectively improve the clinical diagnosis and treatment ability of students.
