经宫腔镜输卵管栓堵术治疗输卵管积水的临床效果分析

摘　　要：目的:分析经宫腔镜输卵管栓堵术治疗输卵管积水的临床效果。方法:选取2020年1月—2023年1月昆明医科大学第二附属医院生殖医学科收治的因输卵管积水行体外受精-胚胎移植(IVF-ET)的患者78例为研究对象,采用随机数字表法分为对照组与观察组,每组39例。对照组行腹腔镜下输卵管结扎术,观察组行宫腔镜输卵管栓堵术。比较两组手术指标、卵巢功能指标、术后IVF-ET助孕相关指标水平及IVF-ET结局。结果:观察组手术时间、术后观察时间均短于对照组,术中出血量少于对照组(P<0.001)。术后,对照组卵泡计数降低,且低于同时期观察组(P<0.05)。两组术后IVF-ET助孕相关指标水平比较,无统计学差异(P>0.05)。观察组输卵管妊娠率低于对照组(P=0.015)。结论:采取经宫腔镜输卵管栓堵术治疗输卵管积水效果较好,手术时间短、创伤小,术后恢复快,可改善患者的卵巢功能,降低输卵管妊娠发生率,且不会对超排卵指标产生不利影响。Objective:To analyze the clinical effect of hysteroscopic salpinx plugging in the treatment of hydrosalpinx.Methods:A total of seventy-eight patients who underwent in vitro fertilization-embryo transfer(IVF-ET)due to hydrosalpinx admitted to the Department of Reproductive Medicine,the Second Affiliated Hospital of Kunming Medical University from January 2020 to January 2023 were selected as the study objects,and were divided into control group and observation group by random number table method,with thirty-nine cases in each group.The control group underwent laparoscopic tubal ligation,and the observation group underwent hysteroscopic salpinx plugging.The levels of surgical indexes,ovarian function indexes,post-operative IVF-ET pregnancy-related indexes and IVF-ET outcomes were compared between the two groups.Results:The operation time and postoperative hospital stay of the observation group were shorter than those of the control group,and the amount of blood loss during operation was less than that of the control group(P<0.001).After operation,the follicle count of control group was lower than that of observation group(P<0.05).There was no statistically significant difference in the levels of postoperative IVF-ET assisted pregnancy related indicators between the two groups(P>0.05).The tubal pregnancy rate in the observation group was lower than that in the control group(P=0.015).Conclusion:The treatment of hydrosalpinx by hysteroscopic salpinx plugging has good effect,short operation time,less trauma and quick postoperative recovery,which can improve the ovarian function of patients,reduce the incidence of tubal pregnancy,and will not adversely affect the index of superovulation.
