摘　　要：目的:探讨腹腔镜辅助下前后联合入路手术治疗腰椎爆裂骨折的效果。方法:选取2020年4月—2023年5月白银市第一人民医院收治的腰椎爆裂骨折患者60例为研究对象,采用随机数字表法分为对照组与观察组,每组30例。对照组使用一期后路减压重建术,观察组使用腹腔镜辅助下前后联合入路手术。比较两组手术指标、并发症发生情况、脊柱解剖指标。结果:观察组手术时间、住院时间均长于对照组,术中出血量、术后引流量均大于对照组(P<0.001);两组并发症总发生率比较,差异无统计学意义(P>0.05);术后1周,两组伤椎前缘相对高度、伤椎后缘相对高度增大,观察组大于对照组,后凸Cobb角减小,观察组小于对照组(P<0.05)。结论:应用腹腔镜辅助下前后联合入路手术治疗腰椎爆裂骨折,能提高脊柱解剖结构恢复效果,不会增加并发症发生风险,但手术时间长、创伤大,术后恢复较慢。Objective:To investigate the effect of laparoscopic combined anterior and posterior approach in the treatment of lumbar burst fractures.Methods:A total of 60 patients with lumbar burst fracture treated in Baiyin First People's Hospital from April 2020 to May 2023 were selected as the study objects,and were divided into control group and observation group by random number table method,with 30 cases in each group.The control group underwent one-stage posterior decompression and reconstruction,and the observation group underwent laparoscopic-assisted anterior-posterior combined approach.The surgical indexes,complications and spinal anatomic indexes were compared between the two groups.Results:The operation time and hospital stay in the observation group were longer than those in the control group,and the intraoperative blood loss and postoperative diversion volume were greater than those in the control group(P<0.001).There was no significant difference in the total incidence of complications between the two groups(P>0.05).One week after surgery,the relative height of the anterior edge of the injured vertebra and the posterior margin of the injured vertebra increased in both groups,and the kyphotic Cobb Angle decreased in the observation group than in the control group(P<0.05).Conclusion:Laparoscopic combined anterior and posterior approach can improve the recovery effect of spinal anatomy without increasing the risk of complications,but the operation time is long,the trauma is large,and the postoperative recovery is slow.
