儿童胸部数字化X线摄影图像质量的影响因素分析

摘　　要：目的:分析影响儿童胸部数字化X线摄影(DR)图像质量的相关因素。方法:将2022年12月—2024年6月于威宁彝族回族苗族自治县妇幼保健院行胸部DR检查的患儿的影像学图像5000张纳入研究。比较两组相关资料,采用多因素Logistic回归分析儿童胸部DR图像质量的独立影响因素。结果:5000张儿童胸部DR检查图像中,优片4504张,良片231张,差片265张,优良片率为94.70%。优良片组4735例,差片组265例。优良片组站立后前位、身上无异物、曝光适当、应用后处理技术、检测医师工作年限>5年占比大于差片组(P<0.001)。多因素Logistic回归分析显示,站立后前位、后处理技术、检测医师工作年限>5年是儿童胸部DR检查影像学图像质量的独立保护因素(P<0.05);身上异物、曝光过度是儿童胸部DR检查影像学图像质量的独立危险因素(P<0.05)。结论:受检体位、身体异物、曝光参数设置、后处理技术应用与否、检查医师工作年限均为儿童胸部DR检查影像学图像质量的影响因素。Objective:To analyze the related factors affecting the image quality of digital chest radiography(DR)in children.Methods:Five thousand imaging images of children who underwent chest DR examination in Bijie Weining Yi Hui Miao Autonomous County Maternal and Child Health Hospital from December 2022 to June 2024 were included in the study.Multivariate Logistic regression was used to analyze the independent influencing factors of DR image quality in children's chest.Results:Among the 5000 DR images of children's chest,4504 were excellent,231 were good,265 were poor,and the rate of excellent was 94.70%.There were 4735 cases in excellent group and 265 cases in poor group.The proportion of excellent group with standing back and forward position,no foreign body,proper exposure,application of post-processing technique,and working years of doctors>5 years was higher than that of poor group(P<0.001).Multivariate Logistic regression analysis showed that the standing back and forward position,post-processing technique and working years of doctors were independent protective factors for the image quality of children chest DR examination(P<0.05).Body foreign body and overexposure were independent risk factors for DR imaging quality in children chest(P<0.05).Conclusion:Body position,foreign body,exposure parameter setting,post-processing technique application or not,and the working years of doctors were all factors influencing the image quality of children chest DR examination.
