不同子宫颈上皮内瘤变患者下生殖道性传播微生物感染特点及与HPV感染关系研究

摘  要:目的探讨不同子宫颈上皮内瘤变(CIN)患者下生殖道性传播感染(STIs)微生物的分布特点及其与人乳头瘤病毒(HPV)感染的关系。方法选取2022年3月至2024年3月于河北省廊坊市人民医院行子宫颈活组织检查确诊为CIN的患者102例,按照CIN分级分为CINⅠ组(52例)、CINⅡ组(31例)、CINⅢ组(19例)。分别对每组进行STIs病原体及HPV分型检测,并对结果进行分析。结果3组CIN患者STIs病原体感染率依组递增,但差异无统计学意义(P>0.05);不同感染类型感染率由低到高为单一、多重、双重感染,提示CIN级别越高,STIs感染率越高,且以混合感染为主;UU、Mh、Mg及双重、多重感染率在HPV阳性、阴性2组间差异有统计学意义(P<0.05),NG、CT、HSV-Ⅱ及单一感染率在2组间差异无统计学意义(P>0.05),提示UU、Mh、Mg及STIs病原体混合感染与HPV感染具有一致性;UU、Mh、Mg及双重、多重感染是HPV感染的危险因素(P<0.05);STIs病原体合并HPV感染率在3组CIN患者间差异无统计学意义(P>0.05),提示STIs病原体感染与HPV感染所致CIN的整个病理过程无关。结论CIN级别越高,STIs微生物感染率就越高,且STIs以混合感染为主,UU、Mh、Mg及STIs病原体混合感染是HPV感染的危险因素。Objective To investigate the distribution characteristics of microbial pathogens involved in sexually transmitted infections(STIs)in the lower genital tract of patients with different grades of cervical intraepithelial neoplasia(CIN)and its relationship with human papillomavirus(HPV)infection.Methods A total of 102 patients diagnosed with CIN by cervical biopsy in Langfang People's Hospital from March 2022 to March 2024 were selected and divided into CIN I group(52 cases),CIN II group(31 cases),and CIN III group(19 cases)according to CIN classification.The pathogen detection of STIs,HPV genotyping were carried out for each group respectively,and the results were analyzed.Results The infection rate of STIs pathogens in the three groups of CIN patients increased gradually with the group,but the difference was not statistically significant(P>0.05).The infection rate of different infection types from low to high was single,multiple,and double infection,suggesting that the higher the CIN grade,the higher the STIs infection rate,and mainly mixed infections;There were statistically significant differences in the infection rates of UU,Mh,Mg and double and multiple infections between the HPV-positive and HPV-negative groups(P<0.05),and there was no statistically significant difference in the infection rates of NG,CT,HSV-II and single infection between the two groups(P>0.05),suggesting that the infections of UU,Mh,Mg and mixed STIs pathogens were consistent with HPV infection;The infection rates of UU,Mh,Mg and double and multiple infections between the HPV-positive and HPV-negative groups were statistically significant(P<0.05),suggesting that the infections of UU,Mh,Mg and mixed STIs pathogens were risk factors for HPV infection;There was no statistically significant difference in the combined infection rate of STIs pathogens and HPV among the three groups(P>0.05),suggesting that STIs pathogen infection was not related to the entire pathological process of CIN caused by HPV infection.Conclusion The higher the grade of cer
