摘 要:目的 评估心脏超声联合颈动脉超声在冠心病诊断中的应用价值。方法 选择2020年5月至2023年12月长安医院确诊的冠心病患者102例作为观察组,募集等量健康志愿者作为对照组。比较心脏超声、颈动脉超声单用或联用对冠心病检出率的差异;对比观察组与对照组在颈动脉超声指标,即颈动脉内膜-中层厚度(IMT)和斑块积分的差异,以及在心脏超声指标,即左心房内径(LA)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)的差别。将冠心病患者按血管病变支数(单支、双支、3支)分组,分析病变血管支数与前述指标的相关性。结果 心脏超声对冠心病的检出率为64.71%,颈动脉超声的检出率40.20%,2种超声联合应用对冠心病的检出率可达77.45%。与对照组相比,观察组的IMT、斑块积分、LA、LVESD、LVEDD明显升高,LVEF明显降低,差异有统计学意义(P <0.05)。冠心病患者的病变血管支数与IMT、斑块积分、LA、LVESD、LVEDD呈正相关,与LVEF呈负相关(P <0.05)。结论 与单用心脏超声或颈动脉超声比较,2种手段联用对冠心病的检出效果最佳。Objective Evaluate the practical ability cardiac ultrasound combined with carotid ultrasound in the diagnosis of coronary heart disease,and provide reliable support for clinical examination of coronary heart disease.Methods A total of 102 coronary heart disease patients diagnosed between May 2020 and December 2023 were enrolled as the observation group,and recruit an equal number of healthy volunteers as the control group.Patients in the observation group were categorized based on the number of coronary artery lesions(single,double,or triple branches),and the parameters such as IMT,Plaque score,and cardiac ultrasound indicators were analyzed.Results(1)Compared with the control group,the observation group showed an increase in IMT,Plaque score,LA,LVESD,and LVEDD,and a decrease in LVEF,with significant statistical differences(P<005).(2)According to the number of coronary artery branch lesions,IMT,Plaque score,LA,LVESD,and LVEDD increased in the order of single,double and triple branch lesions,but LVEF decreased with same order.The test showed significant statistical differences(P<005).(3)After ultrasound examination,the coronary angiography was used to review,the detection rates of cardiac ultrasound,carotid ultrasound,and combined ultrasound were 64.71%,40.20%and 77.45%.The results show that the detection rate of combined ultrasound is the highest.Conclusion The combined strategy of cardiac ultrasound and carotid ultrasound has the best efficiency on coronary heart disease and can further develop its diagnostic efficacy.
