皮肌炎患者血清miR-21、LTB4与Th17/Treg平衡的关系

摘　　要：目的 探讨皮肌炎患者血清微小RNA-21(miR-21)、白三烯B4(LTB4)与辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡的关系。方法 选取2021年1月至2024年2月广东省佛山市第一人民医院收治的121例皮肌炎患者为研究对象(皮肌炎组),根据疾病活动程度分为活动期组(n=63)和缓解期组(n=58),另选取同期121例体检健康者为对照组。采用实时荧光定量PCR检测血清miR-21和外周血维甲酸相关孤核受体γt(RORγt)mRNA、叉头框转录因子P3(FOXP3)mRNA水平,酶联免疫吸附法检测血清LTB4、白细胞介素6(IL-6)、IL-10、IL-17A、转化生长因子β1(TGF-β1)水平,流式细胞术检测外周血Th17、Treg比例,并计算Th17/Treg比值。通过Pearson相关系数分析皮肌炎患者血清miR-21、LTB4水平与Th17、Treg相关因子和外周血Th17、Treg、Th17/Treg比值的相关性。结果 与对照组相比,皮肌炎组血清miR-21、LTB4、IL-6、IL-17A、RORγt mRNA水平和外周血Th17比例、Th17/Treg比值均升高,血清IL-10、TGF-β1、FOXP3 mRNA水平和外周血Treg比例均降低(P <0.05);与缓解期组相比,活动期组血清miR-21、LTB4、IL-6、IL-17A、RORγt mRNA水平和外周血Th17比例、Th17/Treg比值均升高,血清IL-10、TGF-β1、FOXP3mRNA水平和外周血Treg比例均降低(P <0.05);Pearson相关性分析结果显示,皮肌炎组患者的血清miR-21、LTB4水平与血清IL-6、IL-17A、RORγt mRNA水平和外周血Th17比例、Th17/Treg比值呈正相关,与血清IL-10、TGF-β1、FOXP3 mRNA水平和外周血Treg比值呈负相关(P <0.05)。结论 皮肌炎患者血清miR-21、LTB4水平升高,可能通过调节Th17/Treg平衡参与皮肌炎的发生发展。Objective To explore the relationship between serum microRNA-21(miR-21),leukotriene B4(LTB4),and the balance of T helper cells 17(Th17)and T regulatory cells(Treg)in patients with dermatomyositis.Methods A total of 121 patients with dermatomyositis,treated in Foshan First People's Hospital from January 2021 to February 2024 were selected as the dermatomyositis group.Based on the disease activity,they were divided into the active phase group(63 cases)and the remission phase group(58 cases).Additionally,121 healthy individuals undergoing physical examination during the same period were selected as the control group.Serum miR-21 levels,RORγt mRNA,and FOXP3 mRNA were detected using real-time quantitative PCR.Serum LTB4,interleukin-6(IL-6),IL-10,IL-17A,and transforming growth factor-β1(TGF-β1)levels were measured by enzyme-linked immunosorbent assay.The proportion of Th17 and Treg in peripheral blood was detected by flow cytometry,and the Th17/Treg ratio was calculated.Pearson correlation coefficient was used to analyze the correlation between serum miR-21,LTB4 levels,and Th17/Treg-related factors,as well as the Th17,Treg proportions,and Th17/Treg ratio in peripheral blood of dermatomyositis patients.Results Compared with the control group,the dermatomyositis group had significantly elevated serum levels of miR-21,LTB4,IL-6,IL-17A,RORγt mRNA,and higher Th17 proportion and Th17/Treg ratio in peripheral blood,while serum levels of IL-10,TGF-β1,FOXP3 mRNA,and Treg proportion in peripheral blood were significantly decreased(P<0.05).Compared with the remission phase group,the active phase group showed elevated serum levels of miR-21,LTB4,IL-6,IL-17A,RORγt mRNA,and increased Th17 proportion and Th17/Treg ratio,along with decreased serum levels of IL-10,TGF-β1,FOXP3 mRNA,and Treg proportion in peripheral blood(P<0.05).Pearson correlation analysis showed that in dermatomyositis patients,serum miR-21 and LTB4 levels were positively correlated with serum IL-6,IL-17A,RORγt mRNA levels,and Th17 proportion and Th17/Treg ra
