摘  要:目的探讨经阴道超声检测子宫内膜厚度、子宫动脉血流参数对子宫内膜异位症(EMs)不孕患者子宫内膜容受性(ER)的影响。方法选择2023年3月至2024年3月佛山复星禅诚医院收治的30例EMs不孕患者作为观察组,另外选取同期正常体检的30例育龄期女性作为对照组。所有入选者均进行经阴道超声检查。比较2组子宫内膜厚度、子宫动脉搏动指数(PI)、阻力指数(RI)、收缩期峰值流速与舒张末期流速的比值(S/D);比较2组子宫内膜及内膜下血流分布类型。采用Logistic回归分析经阴道超声检测子宫动脉血流参数对ER低下的影响。结果2组子宫内膜厚度比较差异均无统计学意义(P>0.05),观察组S/D、PI、RI均高于对照组(P<0.05)。2组子宫内膜血流分布类型比较差异有统计学意义(P<0.05),观察组Ⅰ型占比为23.33%,低于对照组的70.00%。经单因素分析显示,观察组PI≥1.6、S/D≥3.6、RI≥0.72占比均高于对照组(P<0.05)。经Logistic回归分析,PI≥1.6、S/D≥3.6、RI≥0.72是EMs不孕患者ER低下的独立危险因素(P<0.05,且OR≥1)。结论子宫动脉PI≥1.6、S/D≥3.6、RI≥0.72是EMs不孕患者ER低下的独立危险因素。Objective To explore the evaluation value of transvaginal ultrasound in measuring endometrial thickness and blood flow parameters for endometrial receptivity(ER)in infertile patients with endometriosis(EMs).Method 30 infertile patients with EMs admitted to Foshan Fosun Chancheng Hospital from March 2023 to March 2024 were selected as the observation group;Additionally,30 women of childbearing age who underwent normal physical examinations during the same period were selected as the control group.All selected individuals underwent transvaginal ultrasound examination.Compare the endometrial thickness,endometrial pulsatility index(PI),S/D,and resistance index(RI)between two groups;Comparing the blood flow distribution types of two groups of endometrium and subendometrium,classification criteria:during the examination,blood flow signals were observed in both endometrium and subendometrium,indicating typeⅠ;only blood flow signals were observed in subendometrium,indicating typeⅡ;and no obvious blood flow signals were observed in both endometrium and subendometrium,indicating typeⅢ.Using logistic regression analysis to investigate the effect of transvaginal ultrasound detection of blood flow parameters on ER deficiency.Result The difference in endometrial thickness between the two groups was not statistically significant(P>0.05);The S/D value,PI,and RI of the observation group were(4.26±0.41),(1.92±0.31),and(0.88±0.06),all higher than those of the control group(3.22±0.34),(1.34±0.22),and(0.71±0.04),with statistical significance(P<0.05).The proportion of type I blood flow distribution in the observation group was 23.33%(7/30),which was lower than the control group's 70.00%(21/30),and the difference was statistically significant(P<0.05).Single factor analysis showed that the proportion of PI≥1.6,S/D≥3.6,and RI≥0.72 in the observation group was higher than that in the control group(P<0.05).According to logistic regression analysis,PI≥1.6,S/D≥3.6,and RI≥0.72 are independent risk factors for ER reduct
