摘 要:目的探索胰岛素抵抗的替代标志物TyG-BMI指数对老年人骨质疏松的诊断价值。方法回顾性分析2023年1月至10月吉林大学中日联谊医院收治的100例骨质疏松症患者及100例非骨质疏松症患者的临床资料。统计2组患者的基线资料及TyG-BMI指数水平、腰椎骨密度,分析TyG-BMI与BMD之间的关联。结果非骨质疏松症组空腹血糖、血清尿酸、肌酐、TyG-BMI均高于骨质疏松症组(P<0.05),而骨质疏松症组甘油三酯高于非骨质疏松症组(P<0.05)。多因素Logistic回归分析表明,TyG-BMI指数与BMD呈显著正相关,且这种关联在调整混杂因素后依然显著(P<0.05)。结论TyG-BMI指数可作为预测老年人骨质疏松风险的有效指标,具有早期筛查的临床应用潜力。TyG-BMI的简便性和经济性使其适合推广应用于老年人群的基础医疗保健中,有助于提高骨质疏松的早期诊断率。Objective To explore the diagnostic value of the TyG-BMI index,a surrogate marker of insulin resistance,for osteoporosis in elderly individuals.Methods This retrospective study analyzed clinical data from 100osteoporosis patients and 100non-osteoporosis patients admitted to the China-Japan Union Hospital of Jilin University between January and October 2023.Baseline characteristics,TyG-BMI index levels,and lumbar bone mineral density(BMD)were compared between the two groups,and the association between TyG-BMI and BMD was analyzed.Results The non-osteoporosis group had higher fasting glucose,serum uric acid,creatinine,and TyG-BMI index levels than the osteoporosis group(P<0.05),while triglyceride levels were higher in the osteoporosis group(P<0.05).Multivariate logistic regression analysis indicated a significant positive correlation between the TyG-BMI index and BMD,and this association remained significant after adjusting for confounding factors(P<0.05).Conclusion The TyG-BMI index may serve as an effective predictor of osteoporosis risk in elderly individuals,offering potential for early screening in clinical settings.Its simplicity and cost-effectiveness make TyG-BMI suitable for broader application in basic healthcare for the elderly,aiding in the early diagnosis of osteoporosis.
