摘  要:目的分析载脂蛋白A5(ApoA5)基因rs662799位点基因型与冠心病(CHD)及血脂水平的相关性。方法回顾性分析新疆医科大学第一附属医院2019年1月至2021年1月住院的CHD患者976例为CHD组,并选择907例非CHD患者为对照组,两组平均年龄为(56.11±10.28)岁和(55.70±9.21)岁,采用SNPscanTM高通量基因分型技术检测所纳入研究对象基因多态性,并检测血脂水平。结果与正常对照组相比,CHD组患者的高血压史(49.3%)、糖尿病史(22.9%)、吸烟史构成比(44.9%)、甘油三酯(TG)(2.04±1.63)mmol/L、血清总胆固醇(TC)(4.46±1.29)mmol/L、低密度脂蛋白胆固醇(LDL-C)(2.84±1.01)mmol/L指标水平更高,高密度脂蛋白胆固醇(HDL-C)(0.99±0.29)mmol/L水平更低,差异均具有统计学意义(均P<0.05)。在CHD组中,rs662799 AG(31.9%,42.5%)和GG(5.9%,8.5%)基因型HDL-C水平明显降低,TG水平明显升高。多因素Logistic回归分析结果显示,CHD组rs662799 GG、GA基因型携带者TG的水平比AA基因型携带者高(OR=0.167,0.184),差异均具有统计学意义(P<0.05)。rs662799 GG基因型携带者HDL-C的水平比GA、AA基因型携带者低(OR=4.027,4.038),差异不具有统计学意义(P>0.05)。结论ApoA5 rs662799影响血脂水平,但与CHD发生发展无明显相关性。Objective To analyze the distribution frequency of apolipoprotein A5(ApoA5)rs662799 genotype in Xinjiang population and its correlation with coronary heart disease(CHD)and blood lipid levels.Methods A total of 976 patients with CHD hospitalized in the First Affiliated Hospital of Xinjiang Medical University from January 2019 to January 2021 were retrospectively analyzed as the CHD group,and 907 patients without CHD were selected as the control group.The average age of the two groups was(56.11±10.28)years old and(55.70±9.21)years old.Using SNPscanTM high-throughput genotyping technology to detect gene polymorphism and blood lipid levels in the included study subjects.Results Compared with the normal control group,the CHD group had higher levels of hypertension history(49.3%),diabetes history(22.9%),smoking history(44.9%),triglyceride(TG)(2.04±1.63)mmol/L,total cholesterol(TC)(4.46±1.29)mmol/L,low-density lipoprotein-C(LDL-C)index(2.84±1.01)mmol/L,and lower levels of high-density lipoprotein-C(HDL-C)(0.99±0.29)mmol/L,and the differences were statistically significant(all P<0.05).In the CHD group,the levels of HDL-C were significantly reduced and TG levels were significantly increased in the rs662799 AG(31.9%,42.5%)and GG(5.9%,8.5%)genotypes.The results of multiple Logistic regression analysis showed that the TG levels in carriers of rs662799 GG and GA genotypes in the CHD group were higher than those in carriers of AA genotype(OR=0.167,0.184),and the differences were statistically significant(P<0.05).The level of HDL-C in carriers of rs662799 GG genotype was lower than that in carriers of GA and AA genotypes(OR=4.027,4.038),and the difference was not statistically significant(P>0.05).Conclusions ApoA5 rs662799 affects blood lipid levels,but there is no significant correlation between the occurrence and development of CHD.
