遵“木郁达之”辨治肢端型白癜风体会

摘　　要：白癜风是一种常见的特发性色素脱失性皮肤病,可发生在全身各个位置,其中肢端型白癜风严重影响患者的社交、工作、婚育等社会活动。中医辨治本病具有重大的临床意义。中医学认为,肝气郁结在肢端型白癜风的发病和进展中占据举足轻重的地位。“木郁达之”出自《黄帝内经》,阐释了肝郁证的病机、治则。本研究根据古医籍,结合临床诊疗,分别介绍“木郁达之”理论的含义和肢端型白癜风病因病机之木郁的特点,以佐证肢端型白癜风的发病病机与肝郁有密切关系,将“木郁达之”理论用于治疗肢端型白癜风,以疏肝解郁为要,配合健脾、活血、补益肝肾法,四逆散为基础方,并重视引经药的使用,以期提高肢端型白癜风的治疗效果,为肢端型白癜风的中医治疗提供新思路。Vitiligo is a common idiopathic depigmented skin disease,which can occur in various locations of the body.Among Vthem,acral vitiligo seriously affects the social life,work,marriage and childbirth,and other social activities of patients.It is of great clinical significance to treat the disease based on traditional Chinese medicine differentiation and treatment.TCM believes that the stagnation of liver Qi(气)plays an important role in the onset and progression of acral vitiligo.The theory of soothing stagnant liver comes from Huangdi Neijing(《黄帝内经》),explaining the pathogenesis and treatment principles of liver depression syndrome.According to ancient medical books and clinical diagnosis and treatment,the author introduces the meaning of soothing stagnant liver theory,the characteristics of stagnant liver as the etiology and pathogenesis of acral vitiligo,respectively,so as to prove that the pathogenesis of acral vitiligo is closely related to liver depression.The theory of soothing stagnant liver is used in the treatment of acral vitiligo,taking the method of soothing liver and relieving depression as the key,cooperating with the methods of invigorating spleen,promoting blood circulation,and tonifying liver,Sini San(四逆散)as the basic prescription,in order to improve the therapeutic effect and provide new ideas and methods for the treatment of acral vitiligo.
